Exploring Traditional Chinese Medicine for Anal Fistula Treatment Options - Traditional Chinese Medicine

Anal fistula

I have been suffering from an anal fistula for three years.
After examinations at National Taiwan University Hospital and Veterans General Hospital, both indicated that surgery is necessary and that it cannot be sutured, so I have been hesitant to undergo the procedure.
However, I experience swelling and pain every two to three weeks, and I usually go to the pharmacy to get anti-inflammatory medication for a few days.
I realize that this is not a long-term solution, and after researching online, I found that Traditional Chinese Medicine offers many treatment options, which is quite encouraging.

I would like to ask:
1.
Is the thread-lifting therapy more effective in curing the condition compared to other methods (oral medication, sitz baths, topical medications, etc.)?
2.
Does thread-lifting therapy carry risks, such as injury to the anal sphincter or surrounding tissues?
3.
Are there laser or other therapies that are less painful and offer quicker recovery than thread-lifting therapy? If so, could you recommend reputable doctors or hospitals?
4.
Can you recommend any well-regarded practitioners of thread-lifting therapy in areas such as Xinzhuang, Banqiao, Taipei City, or Sanchong? Thank you very much!

JuJu, 40~49 year old female. Ask Date: 2009/05/27

Dr. Hou Yuchang reply Traditional Chinese Medicine

Dear JuJu: As far as I know, thread lifting therapy is not widely practiced in Taiwan, so I cannot recommend any doctors for this procedure.
Thread lifting requires a significant amount of time, and the most critical issue is whether the threads can be anchored to the deepest layers; otherwise, there is a high risk of recurrence.
Additionally, the speed of wound healing is also a key factor.
Overall, I do not recommend this therapy.
Wishing you peace and health.
Sincerely, Dr.
Hou Yu-Chang, Attending Physician, Department of Traditional Chinese Medicine, Taoyuan General Hospital, Ministry of Health and Welfare.

When it comes to treating anal fistulas, the options available can vary significantly between Traditional Chinese Medicine (TCM) and Western medicine. Your concerns about the persistent pain and discomfort associated with your anal fistula, as well as the hesitance to undergo surgery, are understandable. Let’s explore the questions you have regarding TCM treatments, particularly the hanging thread therapy (掛線療法), as well as other potential options.

1. Hanging Thread Therapy vs. Other Methods: Hanging thread therapy is a TCM technique that involves placing a thread through the fistula to promote healing. While some practitioners believe it can help in managing symptoms and potentially lead to healing, it is essential to note that the effectiveness of this method can vary widely among individuals. Other TCM approaches, such as herbal medicine, topical applications, and acupuncture, may also provide symptomatic relief. However, there is limited scientific evidence to support the claim that hanging thread therapy can definitively cure anal fistulas compared to surgical options. In many cases, surgery remains the most reliable method for complete resolution.

2. Risks of Hanging Thread Therapy: As with any medical procedure, hanging thread therapy carries risks. There is a possibility of injuring the anal sphincter muscle or surrounding tissues, which could lead to complications such as incontinence or further infection. The success of the therapy largely depends on the skill of the practitioner and the specific characteristics of the fistula. Therefore, it is crucial to consult with a qualified TCM practitioner who has experience in this area.

3. Alternative Treatments: In recent years, laser therapy has emerged as a less invasive option for treating anal fistulas. This method can minimize pain and promote quicker recovery times compared to traditional surgical approaches. Other minimally invasive techniques, such as fibrin glue or seton placement, may also be available. It is advisable to discuss these options with a colorectal surgeon who can provide insights based on your specific condition and medical history.

4. Finding Qualified Practitioners: While I cannot recommend specific practitioners or hospitals, I encourage you to seek out TCM practitioners who specialize in gastrointestinal disorders or anal conditions. Look for practitioners with good reviews and a solid reputation in your area, such as Xinzhuang, Banqiao, Taipei City, and Sanchong. You may also want to consult with a colorectal surgeon who can provide a comprehensive evaluation of your condition and discuss both surgical and non-surgical options.

In conclusion, while TCM offers various approaches to managing anal fistulas, including hanging thread therapy, it is essential to weigh the potential benefits against the risks. Consulting with qualified practitioners in both TCM and Western medicine will provide you with a well-rounded perspective on your treatment options. Your health and comfort are paramount, and finding the right approach that aligns with your needs is crucial. Always prioritize open communication with your healthcare providers to ensure you receive the best care possible.
