What are the differences between circumcision and not circumcising?

Hello, Dr.
Hsu.
My foreskin is quite loose and can be easily retracted.
I clean it daily during my shower, but I’ve noticed that when I pull the foreskin back to expose the glans and maintain that position, it becomes very painful during an erection, as if the glans is being stretched outward.
Is this normal? Do all men need to undergo circumcision? What are the differences between being circumcised and uncircumcised? Additionally, I have a small question: as long as I can achieve an erection and ejaculate, is my sexual function considered normal? Thank you for your response.

Xiao En, 20~29 year old female. Ask Date: 2009/07/29

Hello, not all males necessarily need to undergo circumcision.
In your case, it seems that surgery may not be required unless there is phimosis, difficulty in cleaning, or the presence of lumps or fibrosis; these conditions would warrant consideration for surgery.
In most other cases, surgery is generally not needed.
If there are any unclear points, feel free to ask again, and we can discuss and research further.

Circumcision is a topic that often raises questions regarding its necessity and implications for men's health. The decision to circumcise can be influenced by various factors, including medical, cultural, and personal preferences. Let’s delve into the pros and cons of circumcision, particularly in relation to your concerns about foreskin tightness and pain during erections.

Understanding Circumcision
Circumcision involves the surgical removal of the foreskin, which is the fold of skin covering the glans (head) of the penis. This procedure is commonly performed on newborns in certain cultures and is also done for medical reasons in older children and adults.

Pros of Circumcision
1. Reduced Risk of Infections: Circumcision can lower the risk of urinary tract infections (UTIs) in infants and may reduce the risk of sexually transmitted infections (STIs) in adults, including HIV.

2. Easier Hygiene: Without the foreskin, maintaining hygiene can be simpler, as there is no need to retract the foreskin to clean underneath it.

3. Prevention of Medical Conditions: Circumcision can prevent conditions such as phimosis (where the foreskin cannot be retracted) and balanitis (inflammation of the glans).

4. Cultural and Religious Significance: For many, circumcision holds cultural or religious importance, which can influence the decision to undergo the procedure.

Cons of Circumcision
1. Surgical Risks: As with any surgical procedure, circumcision carries risks such as bleeding, infection, and adverse reactions to anesthesia.

2. Pain and Recovery: The procedure can be painful, and recovery may involve discomfort, especially in older children and adults.

3. Loss of Sensation: Some studies suggest that circumcision may reduce sensitivity in the glans, although this is a topic of ongoing debate.

4. Ethical Considerations: Some argue against circumcision on ethical grounds, particularly when performed on infants who cannot consent.

Your Specific Concerns
Regarding your experience of pain when the foreskin is retracted during an erection, this could indicate a condition known as phimosis, where the foreskin is too tight to be comfortably retracted. It is not uncommon for men to experience discomfort if the foreskin is not sufficiently elastic. If the pain is significant, it may be worth discussing with a healthcare provider, as they can assess whether circumcision or other treatments may be appropriate.

Is Circumcision Necessary?
Not all men need to be circumcised. Many men live healthy lives without any issues related to their foreskin. If you can maintain good hygiene and do not experience recurrent infections or significant discomfort, circumcision may not be necessary. However, if you find that the pain during erections is affecting your quality of life, it may be worth considering.

Understanding Sexual Function
Regarding your question about sexual function, the ability to achieve an erection and ejaculate is generally considered an indicator of normal sexual function. However, sexual health encompasses more than just these two aspects. Factors such as libido, the ability to maintain an erection, and overall sexual satisfaction also play crucial roles. If you have concerns about your sexual health or function, it may be beneficial to consult a healthcare professional for a comprehensive evaluation.

Conclusion
In summary, the decision to circumcise is highly personal and should be made based on individual circumstances, medical advice, and personal beliefs. If you are experiencing pain or discomfort, it is advisable to seek medical attention to explore your options. Remember, maintaining good hygiene and addressing any medical concerns with a healthcare provider is essential for overall sexual health.
