Rusty utility knife laceration - tetanus concerns?
I was cut on my hand by a rusty utility knife.
The wound is 8mm long and about 1mm to 2mm deep, with blood loss not exceeding 1cc.
At the time, I soaked the wound for 20 seconds in a solution made of one teaspoon of healthy salt mixed with 200cc of tap water.
The bleeding has stopped.
To allow for air circulation, I did not bandage it.
It has been 7 hours since the injury, and there is no redness or swelling around the wound.
I currently have a cold.
Should I get a tetanus vaccine?
Hao tong!!, 10~19 year old female. Ask Date: 2009/11/01
Dr. Chen Mengkan reply Family Medicine
Hello: Whether or not to receive a tetanus vaccine is not directly related to the current state of inflammation in the wound.
Generally, if you have not received the vaccine in the past ten years, it is still recommended to get vaccinated after an injury.
As for the current condition of the wound, the absence of redness and swelling indicates that there is no bacterial infection at this time.
For wound care, you should first apply pressure to stop the bleeding, then clean the wound with saline, and finally apply povidone-iodine to the wound.
Your method of treatment may increase the risk of infection and could be very painful.
Wishing you a speedy recovery.

When it comes to cuts from rusty objects, particularly a rusty utility knife, it is important to consider a few factors regarding tetanus vaccination. Tetanus is a serious bacterial infection caused by Clostridium tetani, which can enter the body through wounds, especially those that are deep or contaminated.
In your case, you mentioned that the cut was approximately 8mm long and 1-2mm deep, with minimal bleeding. You also took immediate action by cleaning the wound with saline solution, which is a good first step in wound care. The fact that there is no redness or swelling around the wound after 7 hours is a positive sign, indicating that there may not be an infection at this point.
Now, regarding the need for a tetanus shot, the general guidelines recommend that adults should receive a tetanus booster every 10 years. However, if you have a wound that is particularly dirty or caused by a rusty object, the recommendation may change. If your last tetanus shot was more than 5 years ago and the wound is considered to be at risk for tetanus (which can be the case with rusty objects), it is advisable to get a booster shot.
Since you mentioned that you have a cold, it is still safe to receive a tetanus shot; however, if you are feeling unwell or have a fever, it might be best to wait until you are feeling better. The vaccine will not interfere with your recovery from a cold, but your body may be less responsive to the vaccine if it is already fighting off an infection.
In summary, here are the key points to consider:
1. Last Tetanus Shot: If your last tetanus booster was more than 5 years ago, you should consider getting a booster, especially since the cut was from a rusty utility knife.
2. Wound Condition: Since the wound appears to be clean and there are no signs of infection, immediate concern may not be necessary, but monitoring the wound for any changes is important.
3. Consult a Healthcare Provider: It is always best to consult with a healthcare provider regarding your specific situation. They can assess the wound and your vaccination history to provide personalized advice.
4. Wound Care: Continue to keep the wound clean and dry. If you notice any signs of infection (increased redness, swelling, warmth, pus, or increased pain), seek medical attention promptly.
5. General Health: If you are feeling unwell due to your cold, focus on recovery, but do not hesitate to address the tetanus vaccination as soon as you are able.
In conclusion, while your wound seems to be healing well, it is prudent to check your tetanus vaccination status and consider a booster if necessary. Your health and safety should always come first, so do not hesitate to reach out to a healthcare professional for further guidance.
