Prostate issues and insufficient blood flow problems?
Hello Dr.
Hsu,
I have been troubled by prostate issues for the past six months, which have significantly affected my quality of life and sleep.
I find myself waking up 2 to 3 times at night, and during the day, I feel the need to urinate almost every hour.
Additionally, it takes me a long time to urinate, about a minute, and I often feel like I haven't completely emptied my bladder.
I have seen doctors multiple times and have tried various medications, but the effects have not been significant.
Could this situation affect the sensation of congestion below? Is it true that male sexual dysfunction cannot be treated? I often feel that when I visit the hospital, I do not receive effective treatment for my symptoms.
The doctors seem indifferent, listening only superficially, and they often prescribe medications without thoroughly addressing my concerns, leaving me with no improvement.
There are so many advertisements for related medications outside that make it hard to trust and leave me feeling helpless.
About three months ago, I took testosterone supplements (DHEA TOFIPAN-Z) in hopes of improving hormonal deficiencies, but after three months, I have seen no benefits.
In fact, I feel even more fatigued and weak, and there has been no improvement in sexual function.
This is truly painful and disappointing.
I hope Dr.
Hsu can provide some guidance.
I am very grateful for your help.
Thank you in advance!

Dr. Xu Weikai reply Urology
Hello, there are many medications available for sexual function, but they all have some side effects to varying degrees.
Currently, the most popular ones are Viagra and Cialis, which have an effectiveness of about 80%, making them relatively stable with well-known side effects.
As for general health supplements, their effects cannot be seen in a short period, and these supplements are not classified as medications.
The ingredients and quality control vary among manufacturers, and since they are considered food products, they are not regulated by pharmaceutical authorities.
Therefore, I can only suggest finding reputable manufacturers to ensure adequate ingredient content.
As for how long it takes to be effective, I cannot provide an answer.
If you need consultation regarding sexual function, it is advisable to visit a urologist who specializes in this area for a more professional diagnosis and treatment.
Note that most medical centers have specialized clinics for sexual function, and consulting these physicians is recommended.
Additionally, since these are specialized clinics, they usually limit the number of patients, ensuring high-quality care.
If there are any further questions or if you would like to discuss this topic more, feel free to ask.
Sincerely, Dr.
Hsu Wei-Kai, Director of Urology, Taitung Hospital, Department of Health, Executive Yuan.

More Info
Hello! Thank you for sharing your concerns regarding prostate issues and their impact on your overall health. It sounds like you are experiencing a range of symptoms that are affecting your quality of life, including frequent urination, a feeling of incomplete bladder emptying, and concerns about sexual function. Let's break down your situation and explore potential causes, implications, and treatment options.
Understanding Your Symptoms
From your description, it appears that you are dealing with lower urinary tract symptoms (LUTS), which can be associated with conditions such as benign prostatic hyperplasia (BPH) or chronic prostatitis. These conditions can lead to symptoms like:
1. Frequent Urination: Needing to urinate multiple times during the night (nocturia) and during the day.
2. Urgency: A sudden, strong need to urinate.
3. Weak Urine Stream: Difficulty starting urination or a weak stream.
4. Incomplete Emptying: The sensation that the bladder is not fully emptied after urination.
These symptoms can significantly impact your daily life, including sleep quality and overall well-being.
Potential Causes
1. Benign Prostatic Hyperplasia (BPH): This is a common condition in older men where the prostate gland enlarges, leading to urinary symptoms. It is not cancerous but can cause significant discomfort and inconvenience.
2. Chronic Prostatitis: This condition involves inflammation of the prostate and can lead to pain and urinary issues. It may be caused by bacterial infections or other factors.
3. Hormonal Imbalances: You mentioned taking testosterone supplements (DHEA and TOFIPAN-Z) without noticeable improvement. Hormonal imbalances can affect libido and erectile function, and low testosterone levels may contribute to fatigue and decreased sexual desire.
Impact on Sexual Function
Your concerns about whether these urinary issues could affect sexual function are valid. Conditions like BPH and prostatitis can lead to erectile dysfunction (ED) due to several factors:
- Physical Discomfort: Pain or discomfort in the pelvic region can make sexual activity less enjoyable or even painful.
- Psychological Factors: Anxiety about urinary symptoms can lead to performance anxiety, which can further exacerbate ED.
- Hormonal Factors: Low testosterone levels can directly impact libido and erectile function.
Treatment Options
1. Medications: Alpha-blockers (like tamsulosin) and 5-alpha-reductase inhibitors (like finasteride) are commonly prescribed for BPH. These can help relieve urinary symptoms by relaxing the bladder neck and reducing prostate size.
2. Lifestyle Changes: Increasing physical activity, maintaining a healthy weight, and reducing caffeine and alcohol intake can help alleviate symptoms.
3. Pelvic Floor Exercises: Kegel exercises can strengthen pelvic floor muscles, potentially improving urinary control and sexual function.
4. Consultation with Specialists: If you feel that your current healthcare providers are not addressing your concerns adequately, consider seeking a second opinion from a urologist who specializes in male health issues. They can perform specific tests to evaluate your prostate health and hormone levels.
5. Counseling: If anxiety or psychological factors are contributing to your symptoms, speaking with a mental health professional can be beneficial.
Conclusion
It’s understandable to feel frustrated when symptoms persist despite treatment. Prostate issues can be complex, and finding the right approach may take time. Open communication with your healthcare provider is crucial, and if you feel unheard, seeking a second opinion can provide new insights and treatment options. Remember, many men experience similar issues, and effective treatments are available. Don’t hesitate to advocate for your health and seek the care you deserve. Thank you for reaching out, and I wish you the best on your journey to better health!
