Circumcision and Infertility

Hello Dr.
Liu: My father and I are both your patients and have consulted with you before.
I have the following questions I would like to ask you, and I hope you can provide your insights.
1.
A few months ago, I came across educational materials regarding women's cervical health.
Besides vaccination, it mentioned the importance of undergoing Pap smears at a certain age.
I also read that if men are circumcised, it can reduce the risk of their partners developing cervical cancer.
Of course, safe and monogamous sexual practices are very important! Additionally, it seems that circumcision may also have some benefits for men themselves.
Therefore, I would like to ask, if I get married in the future, should I consider circumcision for the sake of my partner?
2.
Previously, during my consultation with you (earlier this year), I mentioned that I had "no news" from my ex-girlfriend.
However, time has passed, and she has since married someone else and started a family with children.
So the issue lies with me! At that time, you arranged for me to undergo a semen analysis.
However, I have been busy with living expenses and other factors, and I need to use a DIY method to collect the sample into the plastic container.
I am very worried about the possibility of leakage, insufficient collection, and especially about the discomfort of using this method.
As a result, since the arrangement for the semen analysis earlier this year, I have not submitted the sample to your hospital's laboratory! What should I do? Should I schedule another appointment with you to arrange the test again, or should I wait until I "come to terms" with it and "get used to" the process, then directly submit the sample to your hospital's laboratory and follow up with you regarding the results?
Actually, I have had no "desire" all summer long and have not engaged in any DIY collection.
Besides being busy with income, when I see attractive women, I only "wish" to have a sexual relationship with them.
I have not confessed my feelings or engaged in any sexual activity, let alone the aftermath of DIY collection! I have always been cautious, only engaging in DIY when I can no longer resist.
I believe this is part of maintaining my health.
I often hesitate to confess my feelings to women I like; firstly, I currently cannot afford to support them, and secondly, I am currently unemployed.
I feel anxious! Even if I had a job, I would still hesitate.
One reason is that I am unsure about the other person's personality, and it is likely that our personalities do not match! Secondly, the women I like probably already have partners before I even confess! Just like the lyrics of a certain song, "People are the same; they only like the face of love..."
In the past, I encountered former girlfriends at different ages.
At one point, I found myself in a place with three of my ex-girlfriends, all of whom have families and children now, some even divorced.
Later, we casually discussed who we would choose if we had to select again.
I found it very difficult to decide! Each of my "former" girlfriends had their own "characteristics" (some were very nice, some very knowledgeable, some very beautiful, and some were talented performers, making it feel like "art"...) I struggle internally, feeling a strong desire to "have them all"! I am just a sentimental person; even now, when I see women I like or those with qualities I admire, I "want" to like them.
Sometimes, it gets serious, and I "want" to build a family with them and make them happy! Regardless of whether they are old enough to be my mother or just around 17 or 18 years old.
However, it is rare that I only "want" to engage in a sexual relationship (I haven't done that!).
This truly confirms that "people are the same; they only like the face of love..."
In any case, I know that I need to change; at the very least, I need to change my current lifestyle to see a better future! I appreciate your help in answering the above questions.
Thank you.

Circumcision is a surgical procedure that involves the removal of the foreskin from the penis. This practice has been a topic of discussion regarding its potential health benefits, particularly in relation to sexually transmitted infections (STIs) and certain cancers, including cervical cancer in female partners.
1. Impact on Cervical Cancer Risk: Research indicates that circumcision may lower the risk of cervical cancer in female partners. This is primarily due to the reduced risk of human papillomavirus (HPV) transmission, which is a significant risk factor for cervical cancer. HPV is a sexually transmitted virus that can lead to changes in cervical cells, potentially resulting in cancer. Circumcised men have a lower prevalence of HPV, which can contribute to a decreased risk of cervical cancer in their female partners. Therefore, if you are considering marriage and long-term sexual relationships, discussing circumcision with your partner could be beneficial, especially if there are concerns about HPV and cervical cancer.

2. Personal Health Benefits: For men, circumcision can also provide health benefits. These include a lower risk of urinary tract infections, reduced risk of STIs, and easier hygiene maintenance. Some studies suggest that circumcision may also reduce the risk of penile cancer, although this is a rare condition.
3. Fertility Considerations: Regarding fertility, circumcision itself does not directly impact a man's fertility. However, if a man has recurrent infections or other complications related to an uncircumcised penis, these issues could potentially affect fertility. It’s essential to maintain good genital hygiene and seek medical advice if there are any concerns about infections or other health issues.

4. Addressing Concerns about Semen Analysis: You mentioned concerns about semen analysis and the difficulties you face in collecting a sample. It’s important to remember that semen analysis is a crucial step in evaluating male fertility. If you are uncomfortable with the collection process, consider discussing this with your healthcare provider. They may offer alternative methods for sample collection or provide guidance on how to make the process easier. It’s vital to address any potential fertility issues sooner rather than later, especially if you are planning to start a family.

5. Emotional and Psychological Factors: Your reflections on relationships and emotional struggles are also significant. It’s common to feel overwhelmed by past relationships and the desire for connection. However, focusing on your current situation and what you can control is essential. If you find that your feelings about relationships are affecting your mental health or your ability to pursue a family, consider speaking with a mental health professional. They can provide support and strategies to help you navigate these feelings.

In conclusion, circumcision can have health benefits that may indirectly affect fertility and the health of future partners. If you are considering this procedure, it’s essential to discuss it with your partner and healthcare provider to make an informed decision. Additionally, addressing any concerns about semen analysis and seeking support for emotional challenges can help you move forward in your journey toward starting a family.

Impact of Ignoring Painful Erection Issues on Future Fertility

Hello, doctor. I would like to follow up on my question: if I ignore this and do not seek medical attention, what impact could it have? For example, in terms of future fertility. Thank you.

Dr. Xu Weikai reply Urology
Of course, we can discuss it later. If treatment is not accepted, a tighter foreskin may make cleaning difficult, which could lead to frequent inflammation and subsequently cause foreskin fibrosis (which means it becomes harder). However, if you can take good care of it and there...
