Sexual dysfunction
Hello! I do not have a regular masturbation habit, but I have gradually noticed that I am experiencing erectile dysfunction and premature ejaculation.
My morning erections have also decreased, and while I used to be able to achieve an erection through sexual fantasies, I am currently having difficulty.
Even when I do achieve an erection, it is not very firm and quickly softens.
My daily routine is normal.
Could this be related to having too little sexual activity? Thank you.

Dr. Xu Weikai reply Urology
Hello, at the age of 28, it is unlikely that you would have vascular issues.
The most probable cause is insufficient stimulation leading to incomplete erections.
If there is a strong suspicion of erectile dysfunction, you can undergo tests such as nocturnal penile tumescence testing or penile Doppler ultrasound to confirm the diagnosis.
Of course, these tests are typically available at larger hospitals.
If there are any unclear points, feel free to ask again, and we can discuss and research further.
Sincerely,
Dr.

Erectile dysfunction (ED) and premature ejaculation (PE) are common sexual health issues that can significantly impact a man's quality of life. Understanding the causes and potential solutions for these conditions is crucial for effective management and treatment.
Causes of Erectile Dysfunction
1. Physical Factors: ED can be caused by various physical conditions, including cardiovascular diseases, diabetes, hypertension, and obesity. These conditions can affect blood flow and nerve function, which are essential for achieving and maintaining an erection.
2. Hormonal Imbalances: Low testosterone levels can lead to reduced libido and erectile difficulties. Hormonal changes can occur due to aging, certain medical conditions, or medications.
3. Psychological Factors: Stress, anxiety, depression, and relationship issues can contribute to ED. Psychological factors can create a cycle of anxiety about sexual performance, further exacerbating the problem.
4. Lifestyle Choices: Smoking, excessive alcohol consumption, and lack of physical activity can negatively affect erectile function. These lifestyle factors can lead to vascular problems that impair blood flow to the penis.
5. Medications: Some medications can have side effects that include ED. Common culprits include certain antidepressants, antihypertensives, and medications for prostate conditions.
Causes of Premature Ejaculation
1. Psychological Factors: Anxiety, particularly performance anxiety, can lead to PE. Men who are anxious about sexual performance may ejaculate more quickly.
2. Biological Factors: Hormonal imbalances, neurotransmitter levels, and certain medical conditions can contribute to PE. Some studies suggest that men with higher levels of serotonin may experience longer ejaculatory latency.
3. Relationship Issues: Poor communication with a partner or unresolved relationship conflicts can lead to sexual dysfunction, including PE.
4. Lack of Experience: In some cases, younger men or those with less sexual experience may experience PE due to excitement or anxiety.
Solutions and Treatments
1. Lifestyle Modifications: Improving overall health through regular exercise, a balanced diet, and avoiding smoking and excessive alcohol can enhance sexual function. Weight management can also play a significant role in improving erectile function.
2. Psychotherapy: Counseling or therapy can help address psychological factors contributing to ED and PE. Cognitive-behavioral therapy (CBT) can be particularly effective in managing anxiety and improving sexual performance.
3. Medications: For ED, medications such as phosphodiesterase type 5 inhibitors (e.g., Viagra, Cialis) can be effective. For PE, topical anesthetics or medications that delay ejaculation may be prescribed.
4. Hormonal Treatments: If low testosterone is identified as a cause, hormone replacement therapy may be considered.
5. Pelvic Floor Exercises: Kegel exercises can strengthen the pelvic floor muscles, potentially improving both ED and PE.
6. Communication with Partners: Open discussions about sexual preferences, anxieties, and desires can help alleviate performance pressure and improve sexual experiences.
7. Medical Consultation: It is essential to consult a healthcare provider for a thorough evaluation. They can help identify underlying causes and recommend appropriate treatments tailored to individual needs.
Conclusion
Erectile dysfunction and premature ejaculation are complex issues that can stem from a combination of physical, psychological, and lifestyle factors. Understanding these causes is the first step toward finding effective solutions. If you are experiencing these symptoms, it is advisable to seek medical advice to explore treatment options that can help restore sexual function and improve your overall well-being. Remember, these conditions are common, and seeking help is a positive step toward addressing them.
