Ear problems
Hello, doctor.
I would like to ask you two questions:
1.
I have noticed that when I sleep on my right side, I hear a "ticking" sound in my ear.
At first, I thought there was something in my ear, but there isn't.
The sound only occurs when I sleep on my right side; if I lie flat or sleep on my left side, there is no sound.
This has puzzled me.
I even experimented by completely stilling my body, including my breathing and chest movements, and the sound disappeared.
Could you please help me understand this? Thank you.
2.
Recently, I have noticed that the music I hear in my left ear seems quieter than in my right ear.
I only listen to music for about 15-20 minutes a day while taking the subway.
Is this a sign of potential hearing loss, or could it be that I have too much earwax, possibly impacting my eardrum?

Hello! Thank you for your questions regarding your ear noises and hearing issues. Let's break down your concerns into two parts.
1. The "Didi" Sound When Sleeping on Your Right Side: The sound you are experiencing when lying on your right side could be related to several factors. One possibility is that it is a form of pulsatile tinnitus, which is often described as a rhythmic sound that can be in sync with your heartbeat. This type of tinnitus can sometimes be caused by changes in blood flow or pressure in the ear, which may be more pronounced when you are in certain positions. Another possibility is that the sound is related to the Eustachian tube function. When you lie on one side, the position may affect how air pressure is equalized in your middle ear, leading to sensations or sounds that are not present when you are in a different position.
Additionally, the fact that the sound disappears when you hold your breath or remain still suggests that it may be related to your body's physiological responses. If the sound is bothersome or persistent, it would be wise to consult an ENT specialist who can perform a thorough examination, including hearing tests and possibly imaging studies if necessary.
2. Difference in Sound Perception Between Ears: The observation that music sounds quieter in your left ear compared to your right could indicate a few different issues. One common cause is earwax buildup (cerumen impaction), which can block sound transmission and lead to a perceived decrease in hearing ability. If earwax is pressing against the eardrum, it can dampen sounds and create a sensation of muffled hearing.
Another possibility is that you may be experiencing sensorineural hearing loss, which can affect one ear more than the other. This type of hearing loss can be caused by various factors, including exposure to loud noises, aging, or underlying health conditions. Given that you mentioned listening to music for short periods, it seems less likely that this is the primary cause, but it is still worth considering.
It is also important to note that temporary fluctuations in hearing can occur due to factors such as allergies, sinus congestion, or even changes in atmospheric pressure. If you suspect earwax buildup, it is advisable to have it checked by a healthcare professional who can safely remove any excess wax if necessary.
In summary, both of your concerns warrant further investigation. I recommend scheduling an appointment with an ENT specialist who can conduct a comprehensive evaluation of your ears, including hearing tests and possibly imaging if indicated. They can help determine the underlying causes of your symptoms and recommend appropriate treatment options.
In the meantime, it may be beneficial to avoid exposure to loud noises and to practice good ear hygiene. If you notice any additional symptoms, such as pain, dizziness, or significant changes in hearing, seek medical attention promptly. Your hearing health is important, and addressing these issues early can help prevent further complications.
Wishing you good health and a swift resolution to your concerns!
