Issues related to anal fistulas?

Hello, Doctor: I have a problem with an anal fistula, and the doctor prescribed me an ointment called Bacitracin-Neomycin Ointment 30g/tube.
The label states that it is for first aid, prevention, and relief of infections from cuts, scrapes, and burns.
Should this ointment be applied when there is bleeding or swelling? Last time I asked you, didn’t you say to use a hemorrhoid ointment? Is this Bacitracin-Neomycin Ointment effective? Can it heal the fistula, or is it just for preventing bleeding or swelling? Will this ointment help prevent the fistula from worsening? I kindly ask for your assistance in answering these questions.
Thank you.

Hello, in response to your question.
To be honest, no matter what ointment is applied, an anal fistula will not heal.
The antibiotic ointment prescribed by your doctor is used to prevent bacterial infection, while the hemorrhoid ointment is for reducing swelling and pain.
These two types of ointments are used in different situations depending on the condition's progression.
I want to reiterate that an anal fistula will not heal without surgical intervention.
I understand that many people are reluctant to undergo surgery, but some conditions cannot be cured without surgical treatment, and unfortunately, an anal fistula is one of them.
I hope that everyone can have a correct understanding of this condition and face it bravely to seek treatment.

When it comes to the treatment of anal fistulas, the use of Bacitracin-Neomycin ointment, commonly known as 利膚軟膏 (Lifus Ointment), can be a point of confusion. This ointment is primarily designed for the prevention and treatment of superficial skin infections resulting from minor cuts, abrasions, and burns. Its active ingredients, Bacitracin and Neomycin, are antibiotics that work to inhibit bacterial growth, which can be beneficial in preventing infections in open wounds.

However, when it comes to anal fistulas, the situation is more complex. An anal fistula is an abnormal connection between the anal canal and the skin surrounding the anus, often resulting from an infection or abscess. The treatment of anal fistulas typically requires more than just topical antibiotics. While Bacitracin-Neomycin ointment may help prevent secondary infections in the area, it is not a definitive treatment for the fistula itself.
The ointment may provide symptomatic relief by reducing the risk of infection, especially if there is any bleeding or swelling. However, it does not address the underlying issue of the fistula. The primary goal in treating an anal fistula is to ensure proper drainage and healing of the fistulous tract, which often requires surgical intervention. Surgical options may include fistulotomy, seton placement, or advancement flap procedures, depending on the complexity and location of the fistula.

Using Bacitracin-Neomycin ointment may help in managing the skin around the fistula, particularly if there are signs of irritation or infection. However, it is essential to understand that this ointment alone will not cure the fistula. It may help prevent further complications, but it is not a substitute for surgical treatment.

If you are experiencing ongoing symptoms related to your anal fistula, such as pain, discharge, or recurrent infections, it is crucial to consult with a healthcare provider who specializes in colorectal surgery. They can evaluate the fistula's condition and recommend an appropriate treatment plan, which may include surgical options.

In summary, while Bacitracin-Neomycin ointment can be useful for preventing infections in the skin surrounding an anal fistula, it is not a cure for the fistula itself. Proper evaluation and treatment by a qualified healthcare professional are essential for effective management of anal fistulas. If you have further questions or concerns about your treatment, please do not hesitate to reach out to your healthcare provider for personalized advice and care.
