Nocturnal penile tumescence issues?
Hello Dr.
Hsu,
In your previous message, you mentioned that my condition seems to be related to insufficient stimulation.
However, during intercourse, when the position changes and the penis is withdrawn from the vagina, it quickly (within 10 to 20 seconds) loses erection.
Does this still relate to insufficient stimulation? Additionally, does the duration of nocturnal erections imply a similar duration of maintaining an erection without additional stimulation? Thank you!

If you have concerns, you can go to various medical centers that have clinics for erectile dysfunction for further examination: tests such as penile Doppler ultrasound, cavernosography (venous leak), and nocturnal penile tumescence testing, among others.
These can help identify the underlying causes.
Speculating here will not provide you with answers; everything is just a hypothesis.

Nocturnal erections, also known as nocturnal penile tumescence (NPT), are a normal physiological phenomenon that occurs during sleep, particularly during the rapid eye movement (REM) phase. These erections are a sign of healthy blood flow and nerve function in the penis, and they typically occur several times a night, lasting anywhere from a few minutes to half an hour. Understanding nocturnal erections can provide insights into a man's sexual health and potential underlying issues.
In your case, the quick loss of erection after changing positions during intercourse may indicate a few possibilities. While it could be related to insufficient stimulation, it could also be influenced by psychological factors, physical fatigue, or even anxiety about performance. If the erection is maintained during nocturnal episodes but not during sexual activity, it may suggest that the physiological mechanisms for achieving an erection are intact, but situational factors during intercourse may be affecting your ability to maintain it.
Regarding your question about the duration of nocturnal erections, it is important to note that the time an erection lasts during sleep does not directly correlate with how long an erection can be maintained during sexual activity. Nocturnal erections occur in a relaxed state, free from the pressures and distractions that can accompany sexual encounters. This means that while you may experience prolonged erections during sleep, the dynamics of sexual activity—such as emotional connection, physical stimulation, and even performance anxiety—play a significant role in how long you can maintain an erection during intercourse.
If you are experiencing difficulties in maintaining an erection during sexual activity, it may be beneficial to explore both physical and psychological factors. Stress, anxiety, and relationship dynamics can all impact sexual performance. If this issue persists, consulting with a healthcare professional or a sex therapist may provide valuable insights and strategies to enhance your sexual health.
In summary, nocturnal erections are a normal and healthy part of male physiology, indicating good blood flow and nerve function. The quick loss of erection during sexual activity could be due to various factors, including insufficient stimulation or psychological influences. It is essential to consider both physical and emotional aspects of sexual health and seek professional guidance if concerns continue.
