Is it possible to not undergo circumcision?
Hello doctor, I have a question.
In a normal state or during an erection, my glans does not expose itself, but I can fully retract the foreskin behind the glans.
However, during an erection, it becomes very painful (there's a feeling of constriction near the glans where the foreskin is tight).
I really do not want to undergo circumcision.
Are there any methods to resolve this issue, such as gradually stretching the foreskin? I hope to receive a response.
Thank you!

Dr. Liu Jianting reply Urology
It seems that surgery is unavoidable.
Please visit a hospital to consult a urologist for an examination.

More Info
When it comes to managing tight foreskin issues, also known as phimosis, there are several options available that may help you avoid circumcision. It's important to understand that phimosis can be a common condition, especially in younger males, but if it persists into adulthood, it can lead to discomfort and complications during sexual activity.
Non-Surgical Solutions for Tight Foreskin
1. Gentle Stretching Exercises: One of the most common non-surgical methods is to gently stretch the foreskin. This can be done by carefully pulling back the foreskin during bathing or while applying a lubricant. Over time, this may help increase the elasticity of the skin. It’s crucial to be gentle and not force the foreskin back, as this can cause pain or injury.
2. Topical Steroid Creams: Applying a topical steroid cream, such as betamethasone, can help reduce inflammation and increase the elasticity of the foreskin. This treatment is usually applied for a few weeks and can be effective in conjunction with stretching exercises. Always consult a healthcare provider before starting any medication.
3. Hygiene Practices: Maintaining good hygiene is essential. Regularly cleaning the area can help prevent infections and other complications associated with phimosis. Make sure to gently clean under the foreskin if it can be retracted without pain.
4. Consultation with a Urologist: If you are experiencing significant pain or discomfort, it is advisable to consult a urologist. They can provide a thorough examination and discuss potential treatments tailored to your specific situation. In some cases, they may recommend a preputioplasty, which is a less invasive surgical option than circumcision.
5. Gradual Dilation: Some individuals have found success with gradual dilation techniques, which involve using devices or tools designed to gently stretch the foreskin over time. This should be done under the guidance of a healthcare professional to avoid injury.
When to Consider Surgery
While many individuals can find relief through non-surgical methods, there are situations where circumcision or other surgical interventions may be necessary. If you experience recurrent infections, severe pain, or if the tightness leads to complications such as paraphimosis (where the foreskin cannot be returned to its normal position), surgical options may be the best course of action.
Conclusion
In summary, while circumcision is a definitive solution for phimosis, it is not the only option. Gentle stretching, topical treatments, and proper hygiene can often alleviate the symptoms associated with a tight foreskin. It is essential to approach this condition with care and seek professional guidance to ensure that you are using the most appropriate methods for your situation. If you have ongoing concerns or if your symptoms worsen, do not hesitate to reach out to a healthcare provider for further evaluation and management.
