Managing Nasal Issues: Tips for Relief and Prevention After Surgery - Otolaryngology

There are many issues with the nose; please help me, doctor. Thank you?

Hello Doctor:
1.
I underwent surgery for turbinate hypertrophy and a deviated septum in September of the year before last (I also have allergy issues).
However, I often experience nasal congestion only on the left side, which worsens during sleep.
The doctor advised me to sleep on my side, specifically on my right side, which does help keep my nose clear.
However, once I lie flat for a while, it becomes congested again.
Sleeping on my side for extended periods also causes discomfort.
Why does sleeping on my side improve nasal congestion, and are there other methods to alleviate it?
2.
I am very concerned about the recurrence of turbinate hypertrophy.
What can I do to maintain my nasal health and prevent it from happening again? Are the nasal irrigation products available on the market effective?
3.
Is it advisable for individuals with nasal issues to avoid consuming cold foods and drinks?
4.
The doctor has prescribed a spray called Fluticasone for me to use, but I find that it only works when I use it consistently.
It contains steroids, and I have already used 2-3 bottles.
I am worried about potential side effects from prolonged use.
Can I continue using this spray? Is it considered a form of maintenance therapy?
Thank you very much for your assistance.

Dear Inquirer,
Based on your literal question, here is the response.
Sleeping on one side can alleviate nasal congestion on that side, which is related to changes in blood flow due to gravity.
There are other methods to relieve nasal congestion, but they do not have significant scientific backing.
Nasal irrigation products are effective primarily for congestion caused by mucus blockage, but their effectiveness is limited for cases involving turbinate hypertrophy.
Given current dietary habits and environmental factors, it is indeed somewhat challenging to avoid cold foods, but doing so can help prevent nasal congestion and runny nose.

Steroid nasal sprays contain steroids that are different from the steroids that typically cause side effects; they are related but have different characteristics.
After all, they are like two brothers born from the same parents—while they share the same last name, blood relation, and upbringing, their personalities, temperaments, and future developments may not be the same.
Thus, although both are steroids, one is used in injections while the other is used in nasal sprays, leading to distinctly different reactions in the human body.
Sincerely,
Dr.

Managing nasal issues, especially after surgical interventions like turbinate reduction and septoplasty, can be challenging. Here are some insights and recommendations based on your concerns:
1. Understanding Nasal Congestion and Sleeping Position: It's common for individuals who have undergone nasal surgery to experience congestion, particularly if they have underlying conditions like allergies. The reason side sleeping can alleviate nasal congestion is due to gravity; when you lie on your side, the nasal passages on the lower side can open up more, allowing for better airflow. However, when you lie flat on your back, gravity can cause mucus to pool in the nasal passages, leading to congestion. If side sleeping causes discomfort, consider using a body pillow to maintain a comfortable position while sleeping.

2. Preventing Turbinate Hypertrophy: To prevent the recurrence of turbinate hypertrophy, it's essential to manage any underlying allergic conditions effectively. Regular use of saline nasal sprays or rinses can help keep the nasal passages moist and clear of allergens. These products are generally safe and can be beneficial in reducing inflammation and preventing the buildup of mucus. Additionally, consider using a humidifier in your home, especially during dry seasons, to maintain moisture in the air, which can help alleviate nasal congestion.

3. Dietary Considerations: While there is no strict rule against consuming cold foods or beverages, some individuals report that cold items can exacerbate their nasal symptoms. If you notice that consuming cold foods leads to increased congestion or discomfort, it may be wise to limit these items. Instead, focus on a balanced diet rich in fruits, vegetables, and omega-3 fatty acids, which can help reduce inflammation.

4. Use of Nasal Sprays: The nasal spray you mentioned, likely containing a corticosteroid, can be effective in reducing inflammation and preventing nasal congestion. While there is a concern about the long-term use of corticosteroids, when used as directed by your physician, the benefits often outweigh the risks. It is crucial to follow your doctor's instructions regarding the frequency and dosage of the spray. If you have concerns about side effects, discuss them with your healthcare provider. They may suggest alternative treatments or adjust your regimen to minimize risks.

5. Consultation with Specialists: If your symptoms persist or worsen, consider consulting an ENT specialist for a thorough evaluation. They can assess your nasal passages and determine if there are any anatomical issues or other factors contributing to your congestion. Additionally, they can provide tailored recommendations based on your specific situation.

6. Managing Allergies: Since you have a history of allergies, it's essential to identify and avoid triggers. This may involve allergy testing to pinpoint specific allergens and implementing strategies such as using air purifiers, keeping windows closed during high pollen seasons, and regularly cleaning your living space to reduce dust and pet dander.

In summary, managing nasal issues post-surgery involves a combination of proper sleeping positions, preventive care for allergies, dietary considerations, and appropriate use of prescribed medications. Regular follow-ups with your healthcare provider will help ensure that your nasal health remains optimal and that any concerns are addressed promptly. If you have further questions or need clarification on any of these points, don't hesitate to reach out to your healthcare provider.
