General Breast Health Examination
What is the general procedure for breast health examinations, and what are the consultation fees? Recently, I visited a hospital in Taipei (referred to as Hospital A) due to breast pain, and the registration fee was NT$350.
The doctor initially noted that my menstrual period was approaching and suggested that it was likely just breast tenderness.
However, I felt it was different from typical tenderness, as I experienced localized pain rather than discomfort throughout the entire breast.
The doctor performed a physical examination and I expressed that the painful area was very tender.
The doctor diagnosed it as a fibrocystic condition and advised me to return for an ultrasound after my menstrual period.
At that point, I had no further questions, but since the doctor also practices at another clinic, I inquired about the fees there.
He mentioned that the first visit for new patients is NT$1200, and follow-up visits for regular patients are NT$800.
He then explained that at Hospital A, the health insurance regulations stipulate that the first visit includes only a physical examination, and an ultrasound is done on the second visit (implying that I would need to register twice, costing NT$700).
He suggested that if I went to his clinic, I could have everything done in one visit.
I don't understand why health insurance has such regulations.
My mother had a consultation in Kaohsiung, where the registration fee was NT$200, and she had both the physical examination and ultrasound done in one visit.
Am I being misled?

Dear Miss ypcasper,
Generally, the examination for breast tumors includes physical examination (palpation), ultrasound, and mammography.
The first two can typically be completed in an outpatient setting and are covered by health insurance.
Mammography can also be done on the same day, but the report may take a few days to be ready for follow-up.
Whether it is covered by health insurance can sometimes depend on the situation; for individuals aged 45 to 70, it qualifies for the free screening provided by the National Health Administration every two years.
The examination and fee structure you mentioned may vary by hospital, so you can communicate with the hospital to understand better.
Most medical institutions should adjust according to the different needs of patients.
Based on your described condition, fibrocystic changes are most likely, and better examination conditions can be achieved after the menstrual cycle ends!
Wishing you peace and health,
Dr.

More Info
In Taiwan, breast health check procedures typically involve a series of examinations aimed at assessing breast health and identifying any potential issues. The standard procedure generally includes a clinical breast examination (CBE), which is a physical examination performed by a healthcare professional, and may also involve imaging tests such as mammography and ultrasound, depending on the patient's age, risk factors, and specific symptoms.
The costs associated with these procedures can vary significantly depending on the healthcare facility, the specific tests performed, and whether the patient is using the National Health Insurance (NHI) system. For instance, as you mentioned, the consultation fee at A Hospital was NT$350, which is a common fee for an initial visit. Under the NHI system, the first visit typically includes a clinical breast examination, and if further imaging is required, such as an ultrasound, a follow-up appointment is usually necessary, which incurs additional costs.
In your case, the physician's initial assessment suggested that the localized pain you were experiencing might be due to a fibrocystic change, which is a common condition that can cause breast pain and discomfort. The recommendation to return for an ultrasound after your menstrual cycle is a standard practice, as hormonal changes can affect breast tissue and may provide clearer imaging results post-menstruation.
Regarding the differences in costs and procedures between various hospitals and clinics, it is important to note that while the NHI provides coverage for many essential health services, specific regulations may dictate how and when certain tests are performed. For example, the NHI may require that a clinical examination be conducted before imaging tests like ultrasounds can be billed under insurance. This could explain why you were advised to return for a second visit for the ultrasound, as opposed to having both the examination and imaging done in one visit.
In contrast, private clinics, such as the one operated by your physician, may have more flexibility in their pricing and procedures. The initial fee of NT$1200 for new patients and NT$800 for follow-ups reflects the private sector's ability to set their own prices, which can sometimes lead to a more streamlined process for patients. However, this does not necessarily mean that the NHI system is inadequate; rather, it reflects the differences in how public and private healthcare systems operate.
As for your mother's experience in Kaohsiung, where she was able to receive both a clinical examination and an ultrasound in one visit for a lower fee, this may be due to the specific policies of that hospital or the availability of resources. Each healthcare facility may have different protocols based on their operational capabilities and the patient population they serve.
In summary, breast health checks in Taiwan involve a combination of clinical examinations and imaging tests, with costs varying based on the facility and whether the patient is using NHI. It is advisable to communicate with healthcare providers to understand the specific procedures and costs involved, as well as to inquire about any potential discrepancies in services offered at different facilities. If you have further concerns about your breast health or the costs associated with your care, consider seeking a second opinion or discussing your options with your healthcare provider.
