Irregular menstruation
I have been experiencing irregular menstrual cycles since my first period.
Sometimes I have two periods in one month, and other times my period is delayed for almost two weeks.
I have tried both Western and traditional Chinese medicine to regulate my body, and when I see a doctor and take medication, my cycle becomes normal.
However, when I stop seeing the doctor, my cycle becomes irregular again.
Additionally, during my visits to the doctor at Zhongshan, I often have cystic formations.
What steps can I take to determine the cause of these issues and why my condition is not improving?

Dr. Dong Yuhong reply Obstetrics and Gynecology
Hello: Irregular menstruation can have many causes, most of which are related to ovarian dysfunction.
Medication for regulation may require a continuous period of time.
It would be beneficial to consult your attending physician for advice, as a coordinated treatment plan over time may yield better results.
Thank you.

Irregular menstrual cycles can be a source of concern for many women, and understanding the underlying causes is essential for effective management. Based on your description, it seems that you have experienced significant fluctuations in your menstrual cycle, including instances of having two periods in one month or prolonged delays. This irregularity can stem from various factors, and it is crucial to explore these in detail.
Firstly, hormonal imbalances are one of the most common reasons for irregular periods. The menstrual cycle is regulated by a complex interplay of hormones, including estrogen and progesterone. When these hormones are out of balance, it can lead to irregularities such as missed periods, heavy bleeding, or spotting between periods. Conditions such as polycystic ovary syndrome (PCOS) can also contribute to hormonal imbalances and are often associated with irregular menstrual cycles. PCOS is characterized by the presence of multiple cysts on the ovaries, which can disrupt normal ovulation and lead to irregular periods.
Stress is another significant factor that can affect your menstrual cycle. High levels of stress can impact the hypothalamus, the part of the brain that regulates hormones related to the menstrual cycle. This can result in delayed ovulation or even missed periods. Lifestyle factors such as diet, exercise, and sleep patterns also play a crucial role in menstrual health. For instance, excessive exercise or significant weight fluctuations can lead to irregular cycles.
In your case, you mentioned that you have tried both Western and traditional Chinese medicine to regulate your cycle, with some success when under medical supervision. This suggests that your body may respond positively to treatment when the underlying issues are addressed. However, it is not uncommon for menstrual irregularities to return once treatment is discontinued, especially if the root cause has not been fully resolved.
To investigate the reasons behind your irregular periods further, it would be beneficial to undergo a comprehensive evaluation. This may include:
1. Hormonal Testing: Blood tests can help assess hormone levels, including estrogen, progesterone, and testosterone, to identify any imbalances.
2. Ultrasound: An ultrasound can help visualize the ovaries and uterus, checking for conditions like cysts or fibroids that could be contributing to irregular bleeding.
3. Thyroid Function Tests: Thyroid disorders can also affect menstrual cycles, so evaluating thyroid hormone levels is essential.
4. Lifestyle Assessment: A thorough review of your lifestyle, including stress levels, diet, exercise habits, and sleep patterns, can provide insights into potential contributing factors.
5. Menstrual Diary: Keeping a detailed record of your menstrual cycle, including the timing, duration, and flow of your periods, can help your healthcare provider identify patterns and triggers.
Once the underlying cause is identified, your healthcare provider can recommend appropriate treatment options. This may include hormonal therapies, lifestyle modifications, or other interventions tailored to your specific needs.
In conclusion, irregular menstrual cycles can be caused by a variety of factors, including hormonal imbalances, stress, and lifestyle choices. A thorough evaluation by a healthcare provider is essential to identify the underlying causes and develop an effective treatment plan. It is important to communicate openly with your healthcare provider about your symptoms and concerns to ensure you receive the best possible care. Regular follow-ups and monitoring can help you manage your menstrual health effectively and improve your overall well-being.
