Issues with Wisdom Teeth During Orthodontic Treatment

Hello, doctor.
I am considering orthodontic treatment due to misaligned teeth.
I would like to ask about my upper second molars on both sides, which have been filled due to deep cavities but are functioning normally and have not undergone root canal treatment.
Is it possible to extract them and wait for my wisdom teeth to erupt before using orthodontics to pull them back? I am 30 years old, and I am unsure about the likelihood of wisdom teeth erupting at this age.
Currently, my wisdom teeth have not yet erupted.

sam, 30~39 year old female. Ask Date: 2010/04/26

Dr. Zhuang Suhui reply Dentistry

Hello: Before orthodontic treatment, it is necessary to take X-rays and analyze dental models to determine the method of correction and whether tooth extraction is needed.
X-rays can also help identify the presence of wisdom tooth buds; if there are tooth buds, there is a possibility that wisdom teeth will develop.
Therefore, it is recommended to undergo a pre-orthodontic examination to understand the appropriate course of action.

When considering the management of wisdom teeth during orthodontic treatment, several factors must be taken into account, particularly regarding the status of your existing teeth and the potential for future dental complications.
Firstly, it's important to understand that wisdom teeth, or third molars, typically emerge in late adolescence to early adulthood, usually between the ages of 17 and 25. However, in some cases, they may not erupt at all or may become impacted, meaning they are unable to fully emerge due to lack of space or other dental issues. Given that you are currently 30 years old and your wisdom teeth have not yet erupted, the likelihood of them coming in at this stage is relatively low, but it is not impossible.
If your second molars (the teeth adjacent to your wisdom teeth) have previously been treated for cavities but have not undergone root canal therapy, they may still have a functional role in your dental arch. The decision to extract these teeth before your wisdom teeth have erupted should be carefully considered. If the second molars are healthy and functional, it may be beneficial to retain them, as they play a crucial role in chewing and maintaining the structure of your dental arch.

In orthodontics, the extraction of teeth is sometimes necessary to create space for proper alignment. If your orthodontist believes that the presence of your wisdom teeth could hinder the alignment process or lead to future complications, they may recommend their removal. However, if your second molars are stable and functional, it may be advisable to keep them and monitor the situation with your wisdom teeth.

Additionally, the extraction of teeth can lead to changes in the bite and alignment of the remaining teeth. Therefore, if your orthodontist suggests removing the second molars, they should provide a clear rationale for this decision, including how it will benefit your overall treatment plan.
Regarding the timing of orthodontic treatment, if your wisdom teeth are expected to emerge, it may be wise to wait until they have either erupted or been evaluated for extraction before beginning orthodontic treatment. This can help prevent potential complications, such as crowding or misalignment caused by the wisdom teeth pushing against adjacent teeth.

In summary, the decision to extract your second molars in anticipation of wisdom teeth eruption should be made collaboratively with your orthodontist and possibly a dental surgeon. They can assess the health of your existing teeth, the potential for wisdom teeth eruption, and the overall impact on your orthodontic treatment plan. Regular dental check-ups and X-rays can provide valuable information about the status of your wisdom teeth and help guide your treatment decisions.
Ultimately, maintaining open communication with your dental care team is essential to ensure you receive the best possible outcome for your orthodontic treatment and overall dental health.
