Morning cough

I have a cold, but I almost wake up coughing every morning and always feel an itch in my throat.
Could you please tell me what the reason for this is and what kind of examination I should undergo? Thank you!

Dr. Chen Jianzhi reply Otolaryngology

Dear Inquirer: Hello, based on your literal question, you may ask a physician to examine your nasopharynx or oropharynx for signs of post-nasal drip.
I'm not sure if you have a condition of sinusitis.
Sincerely, Lecturer Chen Jian-Zhi, Fu Jen Catholic University.

More Info

Morning cough can be a common issue, especially when accompanied by symptoms like a scratchy throat. Based on your description, it seems that your cough may be related to several potential causes, including post-nasal drip, allergies, or even gastroesophageal reflux disease (GERD).

Possible Causes of Morning Cough
1. Post-Nasal Drip: This is one of the most common causes of a morning cough. When you lie down at night, mucus can accumulate in the back of your throat due to sinus congestion or allergies. This can lead to irritation and coughing upon waking. If you have a cold, sinus infection, or allergies, this could be exacerbated.

2. Allergies: Allergens such as dust mites, pet dander, or pollen can trigger allergic reactions, especially in the morning when you first wake up. If your bedroom has dust or if you have pets that sleep in your room, this could be a contributing factor.

3. Gastroesophageal Reflux Disease (GERD): GERD can cause stomach acid to flow back into the esophagus, leading to irritation and coughing, particularly when lying down. This is often accompanied by a sensation of heartburn or a sour taste in the mouth.

4. Asthma: If you have a history of asthma or if you experience wheezing or shortness of breath along with your cough, it might be worth considering asthma as a potential cause. Morning cough can be a symptom of poorly controlled asthma.

5. Chronic Bronchitis: If you are a smoker or have been exposed to irritants, chronic bronchitis could be a reason for your cough. This condition is characterized by a persistent cough that produces mucus.

Recommended Tests and Evaluations
Given your symptoms, it would be advisable to consult with a healthcare provider who can perform a thorough evaluation. Here are some tests that may be recommended:
1. Physical Examination: A doctor will likely start with a physical examination, focusing on your respiratory system and throat.

2. Nasal Endoscopy: If post-nasal drip is suspected, a nasal endoscopy may be performed to visualize the nasal passages and sinuses.

3. Allergy Testing: If allergies are suspected, skin or blood tests can help identify specific allergens that may be triggering your symptoms.

4. Pulmonary Function Tests: If asthma or chronic bronchitis is a concern, pulmonary function tests can assess how well your lungs are working.

5. Chest X-ray: This can help rule out any structural issues in the lungs or signs of infection.

6. pH Monitoring: If GERD is suspected, a 24-hour pH monitoring test can help determine if acid reflux is occurring.

Management and Relief
In the meantime, there are several strategies you can employ to alleviate your symptoms:
- Stay Hydrated: Drinking plenty of fluids can help thin mucus and soothe your throat.

- Use a Humidifier: Adding moisture to the air can help relieve throat irritation and reduce coughing.

- Elevate Your Head While Sleeping: This can help prevent mucus from accumulating in your throat and reduce coughing at night.

- Consider Antihistamines: If allergies are suspected, over-the-counter antihistamines may help reduce symptoms, but be sure to choose non-drowsy options if you need to remain alert during the day.

If your symptoms persist or worsen, it’s important to seek medical advice for a proper diagnosis and treatment plan. Early intervention can help prevent complications and improve your quality of life.
