Eczema

Hello, I would like to ask about eczema that is present all over my body.
Recently, I caught a cold and developed a rash on my hands.
Later, I had a scratch on my foot that was healing but has now turned into two small holes with itchy wounds that resemble eczema.
The biggest concern is that the wounds all over my body are not healing and are continuously oozing...
It's very uncomfortable!

Xiao Xiao, 30~39 year old female. Ask Date: 2010/06/11

Eczema, also known as atopic dermatitis, is a chronic inflammatory skin condition that can affect individuals of all ages. It is characterized by dry, itchy, and inflamed skin, and it can appear anywhere on the body. Understanding the causes, symptoms, and effective treatments for eczema is crucial for managing this condition effectively.

Causes of Eczema
Eczema is believed to be caused by a combination of genetic and environmental factors. Individuals with a family history of eczema, allergies, or asthma are at a higher risk of developing the condition. The skin barrier in people with eczema is often compromised, making it more susceptible to irritants, allergens, and microbes. Common triggers include:
1. Irritants: Soaps, detergents, shampoos, and disinfectants can irritate the skin.

2. Allergens: Pollen, dust mites, pet dander, and certain foods can provoke an allergic reaction.

3. Climate: Extreme temperatures, humidity, and sweating can exacerbate symptoms.

4. Stress: Emotional stress can worsen eczema flare-ups.

5. Hormonal Changes: Fluctuations in hormones, particularly in women, can trigger or worsen eczema.

Symptoms of Eczema
The symptoms of eczema can vary from person to person and may include:
- Dry, sensitive skin: The skin may feel rough and tight.

- Intense itching: This can be severe, leading to scratching and further skin damage.

- Red, inflamed patches: These can appear anywhere on the body, often in the creases of the elbows and knees, on the hands, feet, and face.

- Oozing or crusting: In some cases, the affected areas may weep fluid, leading to crusty patches.

- Thickened, leathery skin: Chronic scratching can lead to lichenification, where the skin becomes thick and leathery.

Effective Treatments for Eczema
Managing eczema involves a multi-faceted approach that includes both lifestyle modifications and medical treatments:
1. Moisturizers: Regularly applying emollients or moisturizers helps to maintain skin hydration and restore the skin barrier. Look for products that are fragrance-free and designed for sensitive skin.

2. Topical Corticosteroids: These are commonly prescribed to reduce inflammation and itching during flare-ups. They should be used as directed by a healthcare provider to avoid potential side effects, such as skin thinning.

3. Topical Calcineurin Inhibitors: Medications like tacrolimus and pimecrolimus can be used as alternatives to corticosteroids for sensitive areas, such as the face and eyelids.

4. Antihistamines: Oral antihistamines may help alleviate itching, especially at night.

5. Phototherapy: In some cases, light therapy may be recommended to reduce symptoms.

6. Systemic Medications: For severe cases of eczema that do not respond to topical treatments, systemic medications such as corticosteroids, immunosuppressants, or biologics may be considered.

7. Avoiding Triggers: Identifying and avoiding personal triggers is essential. Keeping a diary of flare-ups can help pinpoint specific irritants or allergens.

Conclusion
Eczema can be a challenging condition to manage, especially when it leads to open wounds and persistent itching. It is important to consult with a dermatologist for a proper diagnosis and tailored treatment plan. In addition, maintaining a consistent skincare routine, avoiding known triggers, and using prescribed medications can significantly improve symptoms and quality of life. If you are experiencing severe symptoms or complications, such as infections from scratching, seek medical attention promptly.
