After hemorrhoid surgery, there is a lump at the anal opening that resembles an external hemorrhoid?
Hello, Doctor.
I underwent a stage III hemorrhoid surgery on June 4.
On the second and third days post-surgery, I noticed a lump at the anal opening resembling an external hemorrhoid, about the size of a fingertip, oval-shaped, measuring approximately 2 cm x 1 cm, with one side close to the surgical wound.
The surgeon mentioned that the swelling was due to blood vessel injury during the procedure and that it would naturally subside in a few weeks.
The surgeon attempted to push it back into the anal canal twice, but it popped out again shortly after.
Now, three weeks have passed, and the surgical wound has begun to heal (there's no pain during bowel movements), but the lump has only changed from red and swollen to a lighter red color, and its length remains the same, having only shrunk about 10-15% (I have been taking regular photos to monitor it).
I am very concerned that it will not resolve as the surgeon indicated.
I also do not understand where this lump came from since it was not present externally before the surgery.
I have a few questions:
1.
Will such a lump generally resolve on its own? If it takes a few weeks, I am willing to wait.
2.
Are there other possibilities or cases where incomplete removal during surgery might necessitate a second surgery?
3.
If a second surgery is required, will the recovery period for the external anal surgery be shorter compared to the hemorrhoid surgery?
Please help me address my concerns.
Thank you.

Hello: In response to your question, the most likely cause of a lump after hemorrhoid surgery is swelling at the surgical site or the accumulation of a hematoma.
If it is swelling, it typically resolves in about two weeks.
The hematoma also tends to resolve in a similar timeframe.
Generally, there is no need for a repeat surgery; simply taking warm sitz baths is usually sufficient.
If a second surgery is truly necessary, the recovery time is generally shorter because the surgical area is more localized.

More Info
After undergoing hemorrhoid surgery, it is not uncommon for patients to experience various post-operative symptoms, including swelling around the anal area. In your case, the presence of a lump resembling an external hemorrhoid, which developed a few days after surgery, can be concerning. However, it is essential to understand the nature of this swelling and what to expect during the recovery process.
1. Will the lump go away?
Typically, post-surgical swelling can occur due to several reasons, including trauma to the blood vessels during the procedure, which can lead to localized inflammation. In many cases, such swelling is temporary and will gradually subside as the healing process progresses. It is not unusual for the lump to take several weeks to diminish, and the timeline can vary from person to person. Since you mentioned that the lump has changed in color and size, albeit slightly, this could be a positive sign indicating that the healing process is underway. It is essential to continue monitoring the lump and maintain communication with your surgeon regarding any changes.
2. Could this be a sign of incomplete removal or a different issue?
While it is possible for a lump to form if there are remnants of hemorrhoidal tissue or if a new issue arises, it is crucial to differentiate between normal post-operative swelling and a complication. If the lump persists beyond the expected recovery period or if you experience increased pain, bleeding, or other concerning symptoms, it would be advisable to consult your surgeon for further evaluation. They may recommend imaging studies or a physical examination to determine the nature of the lump and whether any additional intervention is necessary.
3. If a second surgery is required, what would the recovery time be?
In the event that a second surgery is deemed necessary, recovery times can vary based on the complexity of the procedure and the individual’s overall health. Generally, recovery from a second hemorrhoid surgery may be shorter than the initial surgery, as the tissue may be less inflamed and more amenable to healing. However, this is not a guarantee, and individual experiences can differ significantly. Your surgeon will provide specific guidance based on your unique situation.
In conclusion, while it is understandable to be concerned about post-operative symptoms, many patients experience similar issues that resolve with time. It is crucial to follow up with your healthcare provider, especially if you have ongoing concerns or if the lump does not show signs of improvement. They can provide reassurance, monitor your recovery, and intervene if necessary. Remember to keep track of any changes and maintain open communication with your surgical team to ensure the best possible outcome.
