Persistent diarrhea?
Hello Doctor: Since I returned to Taiwan from mainland China at the end of July, I have been experiencing diarrhea for over two weeks.
Each time I go to the bathroom, it takes more than half an hour, and a few days ago, I spent about an hour in the restroom.
I have intense abdominal pain and have been having frequent bowel movements, to the point where I am sweating profusely, getting chills, and feeling dizzy.
Although I am sweating due to the heat and pain, it is strange that I feel cold all over.
The color of my stool is a darker brownish-yellow, sometimes leaning towards black.
Additionally, due to the continuous diarrhea, my hemorrhoids have swollen significantly, blocking the anal opening, making it somewhat difficult to have a bowel movement.
I am still experiencing diarrhea, and the hemorrhoids cannot be pushed back inside, which is quite distressing.
Furthermore, my body feels hot, but my abdomen feels icy cold, and I am unsure what is happening to me.
I have not sought medical attention because I am uncertain if I should see a doctor and which specialty to consult.
I would also like to ask the doctor what the possible causes of persistent diarrhea might be.
Why do I sometimes feel cold despite the hot weather? Do I need to seek medical care? Thank you!

Dr. Liao Yaopan reply Family Medicine
Hello, regarding your question, it would be best to visit the outpatient clinic for a personal examination by a physician.
There are many factors that can cause abnormal bowel movements, and since you have traveled to other places, it is important to rule out any potential infections.
I recommend that you consult with a family medicine or gastroenterology clinic.
Based on the examination results, further testing and treatment may be necessary.
Thank you for your message!

More Info
Prolonged diarrhea can be a concerning symptom and may indicate a variety of underlying health issues. Given your description of symptoms, including severe abdominal pain, prolonged diarrhea lasting over two weeks, changes in stool color, and the presence of hemorrhoids, it is essential to seek medical attention promptly.
Possible Causes of Prolonged Diarrhea
1. Infectious Causes: One of the most common reasons for prolonged diarrhea, especially after traveling, is an infection. This could be due to bacteria (such as E. coli, Salmonella, or Shigella), viruses (like norovirus or rotavirus), or parasites (such as Giardia). These infections can lead to inflammation of the intestines, resulting in diarrhea.
2. Food Intolerances or Allergies: Sometimes, certain foods can trigger gastrointestinal symptoms. Lactose intolerance or gluten sensitivity can lead to diarrhea, especially after consuming dairy or gluten-containing products.
3. Inflammatory Bowel Disease (IBD): Conditions like Crohn's disease or ulcerative colitis can cause prolonged diarrhea, abdominal pain, and other gastrointestinal symptoms. These are chronic conditions that require ongoing management.
4. Irritable Bowel Syndrome (IBS): This functional gastrointestinal disorder can cause symptoms such as diarrhea, abdominal pain, and bloating. Stress and dietary factors often exacerbate IBS symptoms.
5. Medications: Certain medications, especially antibiotics, can disrupt the normal gut flora and lead to diarrhea. If you have recently taken any medications, this could be a contributing factor.
6. Hemorrhoids: The presence of hemorrhoids can complicate bowel movements, especially if you are experiencing diarrhea. Straining during bowel movements can exacerbate hemorrhoids, leading to pain and discomfort.
When to Seek Medical Help
Given your symptoms, it is advisable to seek medical attention for the following reasons:
- Duration: Diarrhea lasting more than two weeks is considered prolonged and warrants evaluation.
- Severe Symptoms: Severe abdominal pain, especially if it is accompanied by sweating, chills, or dizziness, can indicate a more serious condition.
- Stool Characteristics: The presence of dark or black stools can indicate bleeding in the gastrointestinal tract, which requires immediate medical evaluation.
- Dehydration: Prolonged diarrhea can lead to dehydration, which can be serious. Signs of dehydration include excessive thirst, dry mouth, reduced urine output, and dizziness.
- Hemorrhoid Complications: If your hemorrhoids are causing significant pain or are unable to be reduced, this may require medical intervention.
Recommended Actions
1. Consult a Healthcare Provider: It is crucial to see a doctor, preferably a gastroenterologist, who specializes in digestive disorders. They can perform necessary tests, such as stool tests, blood tests, or imaging studies, to determine the underlying cause of your symptoms.
2. Hydration: Ensure you are staying hydrated. Diarrhea can lead to significant fluid loss, so drinking plenty of fluids, including oral rehydration solutions, is essential.
3. Dietary Adjustments: While waiting for your appointment, consider following a bland diet (BRAT diet: bananas, rice, applesauce, toast) to help manage your symptoms. Avoid dairy, fatty foods, and high-fiber foods until your symptoms improve.
4. Monitor Symptoms: Keep track of any additional symptoms that may arise, such as fever, weight loss, or changes in appetite, as these can provide important information to your healthcare provider.
In conclusion, while it may be tempting to wait and see if symptoms resolve on their own, the combination of prolonged diarrhea, severe abdominal pain, and changes in stool color suggests that medical evaluation is necessary. Early intervention can help prevent complications and lead to a more effective treatment plan.
