Urethral reconstruction surgery
I would like to inquire about urethral reconstruction surgery.
I had a car accident six years ago, which resulted in injuries to my pelvis and urethra.
At that time, a temporary stoma was created, which was closed about six months later.
Although I experienced some narrowing afterward, I was still able to urinate.
Last month, I went to Mackay Memorial Hospital for an examination, and the doctor performed a urethral incision.
After the procedure, a catheter was placed for ten days, but after its removal, I was unable to urinate on the third day.
It seems to be due to adhesions.
Currently, I cannot urinate at all because of severe narrowing, and a catheter cannot be inserted.
Therefore, a suprapubic catheter was placed.
The doctor at Mackay Memorial Hospital advised me to undergo another urethral incision, stating that there is no medication for my condition and that I will need regular urethral dilation.
I just had a urethral incision last month.
What recommendations does the doctor have? I am a 25-year-old male.

Hello, sir: Thank you for your inquiry.
I would like to respond to your question and provide the following advice: If urethral stricture is inappropriate, a dilation procedure is certainly necessary.
However, if the recurrence is too rapid or frequent, then reconstructive surgery should be considered.
This aspect is quite difficult to explain on paper, so I recommend seeking assistance from the Urology Department at Tri-Service General Hospital, as they have extensive experience in this area.
Wishing you peace and happiness.

Urethral reconstruction surgery is a complex procedure that aims to restore normal urinary function in patients who have experienced urethral injury or narrowing, often due to trauma or other medical conditions. In your case, it appears that you have been dealing with complications from a pelvic injury sustained in a car accident six years ago, which has led to significant urethral stricture and subsequent urinary difficulties.
The challenges associated with urethral reconstruction surgery can be multifaceted. First, the surgical procedure itself can be technically demanding, especially if there is significant scarring or tissue loss in the urethra. The risk of complications such as stricture recurrence, infection, and urinary incontinence is present. In your situation, it seems that after your recent urethral incision, you have developed severe narrowing, which has resulted in the inability to urinate normally. This is not uncommon, as scar tissue can form after surgery, leading to further narrowing of the urethra.
The recommendation for periodic urethral dilation is a common approach for managing urethral stricture. This procedure involves the gradual widening of the urethra using specialized instruments. While this can provide temporary relief, it may not be a permanent solution, especially if the underlying cause of the stricture is not addressed. In some cases, repeated dilations may be necessary, and they can be uncomfortable and inconvenient for the patient.
In terms of surgical options, if urethral dilation is not effective, your physician may consider additional surgical interventions. These could include a more extensive urethral reconstruction, which might involve the use of grafts or flaps from other tissues to replace the narrowed segment of the urethra. This type of surgery can be more complex and may require a longer recovery time, but it has the potential to provide a more lasting solution to the problem of stricture.
It's also important to address any concerns you may have regarding the impact of these procedures on your sexual function. While urethral surgery can sometimes lead to complications such as erectile dysfunction or changes in sexual sensation, many patients do not experience significant long-term effects on their sexual health. However, it is crucial to have an open discussion with your urologist about these concerns before proceeding with any surgical intervention.
In summary, urethral reconstruction surgery presents various challenges, particularly in cases with a history of trauma and scarring. While urethral dilation may provide temporary relief, further surgical options may be necessary to achieve a more permanent solution. It is essential to maintain open communication with your healthcare provider, who can guide you through the decision-making process and help you understand the risks and benefits of each potential treatment option. Regular follow-ups and monitoring will also be key in managing your condition effectively.
