Intermittent Hematuria After Prostate Surgery: Causes and Concerns - Urology

Intermittent hematuria?

Hello, my father underwent laser surgery for benign prostatic hyperplasia two years ago.
Since then, his regular follow-ups have been normal, but he occasionally experiences hematuria.
The doctor indicated that all test results were normal, so he prescribed anti-inflammatory medication, which indeed improved the symptoms.
However, is it normal for hematuria to occur intermittently, or could there be other underlying causes that require further evaluation at a different clinic? Additionally, could insufficient sleep or inadequate hydration contribute to this issue? I would appreciate your response.
Thank you!

Mr. Wang, 30~39 year old female. Ask Date: 2010/09/09

Dr. Liu Jiaju reply Urology

Due to the numerous factors that can cause hematuria, it is not solely a prostate issue.
If hematuria continues to recur, it is advisable to return to the outpatient clinic for a comprehensive evaluation and examination for safety.

Intermittent hematuria, or blood in the urine, can be a concerning symptom, especially following prostate surgery such as laser treatment for benign prostatic hyperplasia (BPH). In your father's case, it is important to understand the potential causes of this intermittent hematuria and whether it is a normal occurrence or indicative of an underlying issue.

After prostate surgery, it is not uncommon for patients to experience some degree of hematuria. This can be due to several factors, including irritation of the urinary tract, residual effects of the surgery, or even minor trauma to the bladder or urethra during the procedure. The fact that your father has had regular follow-ups and that the results have been normal is reassuring. However, intermittent hematuria can still occur due to various benign reasons, such as:
1. Irritation or Inflammation: The urinary tract can become irritated due to various factors, including urinary tract infections (UTIs), which can lead to hematuria. In your father's case, the prescription of anti-inflammatory medication suggests that inflammation may have been a contributing factor.

2. Residual Effects of Surgery: Surgical procedures can lead to changes in the urinary tract, including scarring or changes in blood vessel integrity, which may result in intermittent bleeding.

3. Dehydration: Insufficient fluid intake can concentrate the urine, potentially leading to irritation of the bladder lining and subsequent bleeding. Encouraging your father to maintain adequate hydration is essential, as dehydration can exacerbate urinary symptoms.

4. Physical Activity: Strenuous physical activity or heavy lifting can sometimes lead to transient hematuria, especially if the bladder or prostate area is stressed.

5. Other Underlying Conditions: While less common, it is essential to rule out other conditions that could cause hematuria, such as kidney stones, tumors, or other urological issues. If the hematuria persists or worsens, further evaluation may be warranted.

Regarding your question about sleep deprivation and low fluid intake, both can indeed contribute to urinary issues. Lack of sleep can affect overall health and immune function, potentially making one more susceptible to infections, including UTIs. Additionally, inadequate hydration can lead to concentrated urine, which may irritate the bladder and lead to hematuria.

In summary, while intermittent hematuria after prostate surgery can be relatively common and often benign, it is crucial to monitor the situation closely. If your father's symptoms persist, worsen, or are accompanied by other concerning symptoms (such as pain, fever, or changes in urinary habits), it would be prudent to consult with a urologist for further evaluation. Regular follow-ups and open communication with healthcare providers are essential to ensure any potential issues are addressed promptly. Encouraging your father to stay hydrated and maintain a healthy lifestyle can also help mitigate some of these symptoms.
