Regarding phimosis?
Hello Doctor, I am someone with phimosis (tight foreskin), but I clean myself thoroughly every day while showering (I have a personal obsession with cleanliness, so I pay great attention to washing).
What I want to ask is actually not related to hygiene; sometimes when I retract the foreskin, I notice that the glans appears to be somewhat dehydrated (it becomes very dry), but it returns to being smooth when I have an erection.
So I would like to ask: 1.
If I undergo circumcision, will the glans frequently experience dehydration? 2.
If I intentionally expose the glans while sleeping every night, will that be harmful? (For example, causing dehydration or issues with retracting the foreskin, etc.) Thank you for your response, Doctor.

Dr. Xu Weikai reply Urology
ABC: Hello, circumcision will not have any adverse effects; in fact, it may exacerbate dehydration.
Additionally, frequent washing, especially with hot water and cleansers, can lead to dryness of the glans.
It is recommended to use cold water for cleaning.
Prolonged exposure of the glans will definitely cause dehydration.
If you have any further questions, feel free to ask, and we can discuss and research this together.

Phimosis, or the condition of having a tight foreskin that cannot be easily retracted over the glans (the head of the penis), can lead to various concerns regarding hygiene and skin health. Your observations about dryness of the glans when the foreskin is retracted are not uncommon, and it's important to address your questions regarding potential surgical intervention and hygiene practices.
1. Will circumcision lead to persistent dryness of the glans?
After circumcision, the glans is exposed to air and friction, which can initially lead to a different sensation and appearance. Many men report that the glans becomes less sensitive over time as it adapts to being exposed. However, the dryness you are experiencing is likely due to the lack of natural lubrication that the foreskin provides when it is in place. Once circumcised, the glans may continue to experience dryness, but this can often be managed with the application of a gentle moisturizer or lubricant. It's essential to maintain good hygiene, but also to ensure that the skin remains hydrated to prevent irritation or discomfort.
2. Is it harmful to intentionally expose the glans while sleeping?
Exposing the glans by retracting the foreskin during sleep can lead to several issues. While it may not directly cause dryness, it can lead to irritation or discomfort due to friction against bedding or clothing. Additionally, if the foreskin is retracted and becomes stuck, it can lead to a condition called paraphimosis, where the foreskin cannot be returned to its original position, potentially leading to swelling and pain. If you are experiencing dryness or discomfort, it may be a sign that your skin needs more moisture or that the friction is causing irritation.
In terms of hygiene, it is commendable that you maintain a regular cleaning routine. However, it is crucial to be gentle when cleaning the area to avoid irritation. If you are concerned about the dryness of the glans, consider using a water-based lubricant or a gentle, fragrance-free moisturizer to keep the skin hydrated.
If you are contemplating circumcision, it is advisable to discuss your concerns with a healthcare provider who specializes in urology or men's health. They can provide you with a comprehensive understanding of the benefits and risks associated with the procedure, as well as alternative treatments for phimosis, such as topical steroid creams that may help in retracting the foreskin without surgery.
In summary, while circumcision may change the way your glans feels and looks, it can also help alleviate issues related to phimosis. However, it is essential to consider the potential for dryness and to take steps to manage it post-surgery. Always consult with a healthcare professional for personalized advice and treatment options tailored to your specific situation.
