Ejaculation issues?
Hello, I have two questions:
First, every time I am about to ejaculate during masturbation, my sperm seems to get stuck and I feel very uncomfortable.
I have to stop moving before I can slowly ejaculate.
Why does this happen? Could it be due to insufficient thyroid function leading to low testosterone levels, which affects sperm release, or could there be another reason?
Second, most normal men have an upward curvature of the penis when erect, but mine curves downward, and I am only 9 years old.
Could this be due to wearing jeans for a long time? Will this affect fertility?

Dr. Xu Weikai reply Urology
Hello, erection and ejaculation are regulated and controlled by the sympathetic and parasympathetic nervous systems.
Ejaculation disorders require further examination.
Additionally, penile curvature, unless the angle is too severe, typically does not hinder fertilization.
If you have any questions, you can bring these issues for outpatient treatment.
If there are any unclear points, feel free to ask again, and we can discuss and research further.

More Info
Ejaculation issues can be a source of concern for many men, and it's important to understand the potential causes and implications. Based on your description, it seems you are experiencing two distinct issues: difficulty with ejaculation and a curvature of the penis during erection. Let's address each of these concerns in detail.
1. Difficulty with Ejaculation
The sensation of sperm being "stuck" during ejaculation can be frustrating and uncomfortable. This issue may arise from several factors, including physiological, psychological, or hormonal influences.
Physiological Factors:
- Pelvic Floor Muscle Tension: The pelvic floor muscles play a crucial role in ejaculation. If these muscles are overly tense, they may impede the smooth flow of semen. Techniques such as pelvic floor relaxation exercises or biofeedback may help alleviate this tension.
- Prostate Health: The prostate gland contributes to the ejaculation process. Conditions such as prostatitis (inflammation of the prostate) can lead to difficulties in ejaculation. If you experience pain or discomfort in the pelvic area, it may be worth consulting a healthcare professional for an evaluation.
Hormonal Factors:
- Testosterone Levels: You mentioned concerns about thyroid function and testosterone levels. While thyroid issues can affect overall energy and metabolism, testosterone is primarily responsible for libido and sexual function. Low testosterone levels can lead to various sexual dysfunctions, including difficulties with ejaculation. A blood test can help determine your testosterone levels and whether they are within the normal range.
Psychological Factors:
- Anxiety and Stress: Psychological factors, including anxiety about sexual performance or stress, can also impact ejaculation. If you find that anxiety is a recurring theme during sexual activities, consider speaking with a mental health professional who specializes in sexual health.
2. Curvature of the Penis
Regarding the curvature of your penis during erection, it is not uncommon for men to have some degree of curvature. However, if the curvature is significant or causes discomfort, it may be worth investigating further.
Possible Causes of Curvature:
- Peyronie's Disease: This condition involves the development of fibrous scar tissue inside the penis, leading to curvature. It can occur due to trauma or injury to the penis, but the exact cause is often unknown. If the curvature is severe or causes pain during erections, it is advisable to consult a urologist for a proper diagnosis and potential treatment options.
- Anatomical Variations: Many men have a natural curvature, which is usually not a cause for concern. If the curvature does not interfere with sexual function or cause pain, it may simply be a normal anatomical variation.
Impact on Fertility:
- Generally, a slight curvature of the penis does not affect fertility or the ability to conceive. However, if the curvature is severe enough to hinder penetration or cause discomfort for either partner, it may be worth discussing with a healthcare provider.
Conclusion
In summary, the issues you are experiencing with ejaculation and penile curvature can stem from various factors, including physiological, hormonal, and psychological influences. It is essential to approach these concerns holistically and consider consulting with a healthcare professional, such as a urologist or an endocrinologist, who can provide a thorough evaluation and tailored advice based on your specific situation. Addressing these concerns can lead to improved sexual health and overall well-being. Remember, open communication with healthcare providers is key to finding effective solutions.
