White Bumps on the Glans: Is it a Fungal Infection? - Urology

glans penis

Doctor, I have developed a layer of white, keratin-like substance on the glans of my penis.
If I don't do anything about it, it thickens, and I often pick at it! It doesn't hurt or itch much on a regular basis.
It started after I soaked in a hot spring last year! I always use a condom during sexual intercourse.
Initially, it was just a small white spot, but the area has been spreading.
I consulted a urologist, and they said it was a fungal infection and referred me to a dermatologist, who diagnosed it as a Candida infection.
However, after using the medication for a long time, there has been no improvement...
Is it really a fungal infection?

Guji, 20~29 year old female. Ask Date: 2010/10/25

Dr. Xu Weikai reply Urology

Hello, it is recommended to clean the glans regularly without using hot water or any cleaning agents.
Simply rinsing with cold water is sufficient.
Additionally, applying petroleum jelly after cleaning can effectively improve the condition.
If you have any further questions or need clarification, feel free to ask, and we can discuss and research together.

The presence of white bumps or a thickened layer on the glans (the head of the penis) can be concerning, and it is understandable that you are seeking clarity on whether this condition is a fungal infection, particularly one caused by Candida, commonly known as yeast.
From your description, it seems that you have been experiencing a persistent issue that began after exposure to a hot spring. The fact that you have been using condoms during sexual activity is a positive step in preventing sexually transmitted infections, but it does not eliminate the possibility of fungal infections, which can occur due to various factors.

Candida infections, particularly those caused by *Candida albicans*, can manifest as white patches or bumps on the skin, often accompanied by itching or irritation. However, you mentioned that you do not experience significant pain or itching, which can sometimes be atypical for a classic yeast infection. The thickened, keratinized appearance you describe could also suggest a condition known as balanitis, which is inflammation of the glans and can be caused by various factors, including irritation, infections, or even friction.

It is essential to consider that not all white lesions on the glans are due to fungal infections. Other potential causes include:
1. Pearly Penile Papules: These are small, benign bumps that can appear around the corona of the glans and are normal anatomical variations. They are not infectious and do not require treatment.

2. Lichen Sclerosus: This is a skin condition that can cause white patches and is more common in uncircumcised males. It may require specific treatment to manage symptoms and prevent complications.

3. Psoriasis or Eczema: These skin conditions can also cause white patches or scaling on the skin, including the genital area.

4. Balanitis: As mentioned earlier, this inflammation can be due to irritation from soaps, lotions, or even friction from sexual activity.
Given that you have been treated for a suspected fungal infection without improvement, it may be beneficial to seek a second opinion or further evaluation from a dermatologist or urologist who specializes in genital skin conditions. They may recommend a biopsy or other diagnostic tests to determine the exact nature of the lesions.

In the meantime, here are some general recommendations:
- Avoid Irritants: Use mild, unscented soaps and avoid harsh chemicals or products that may irritate the area.

- Keep the Area Dry: Fungi thrive in moist environments, so ensure that the area is kept dry and well-ventilated.

- Avoid Self-Diagnosis and Treatment: While it may be tempting to try over-the-counter antifungal creams or other treatments, it is crucial to have a proper diagnosis before starting any treatment.

In conclusion, while it is possible that your symptoms could be related to a fungal infection, the persistence and nature of the bumps suggest that further evaluation is warranted. Consulting with a healthcare professional who can perform a thorough examination and possibly conduct tests will provide you with the best path forward to determine the cause of your symptoms and appropriate treatment options.
