Anal fistula
Hello Doctor: After discovering an anal fistula last October, I delayed surgery until July of this year due to work commitments.
I followed the post-operative care instructions after the surgery, and during my follow-up appointment in early September, the attending physician said that the recovery was going well and that I no longer needed to return.
During this time, I have been doing warm sitz baths daily.
However, to this day, I still experience a small amount of yellowish discharge with a slight odor.
Is this situation normal? I have researched and found that anal fistulas typically heal within one to two weeks, but it has been nearly four months since my surgery, and I am quite concerned.

Hello: If you still have concerns, it is recommended that you return to the outpatient clinic for a follow-up examination to assess the current healing status of the wound.
Wishing you safety.

After undergoing surgery for an anal fistula, it is understandable to have concerns regarding the recovery process, especially when experiencing ongoing discharge. In your case, having a yellowish discharge with a foul odor several months post-surgery raises valid questions about the healing process.
Firstly, it is important to recognize that the healing time for an anal fistula can vary significantly from person to person. While some patients may experience resolution of symptoms within a few weeks, others may take several months to heal completely. The presence of discharge can be a normal part of the healing process, especially if the surgical site is still in the process of closing up. However, the characteristics of the discharge are crucial in determining whether it is a normal part of recovery or a sign of a complication.
In general, a small amount of clear or slightly yellow discharge can be expected as the body heals. This discharge is often a result of the body's natural healing processes, including the formation of granulation tissue. However, if the discharge is persistent, copious, or accompanied by a strong odor, it may indicate an infection or other complications such as a recurrence of the fistula or an abscess formation. Infections can occur post-surgery, and they may require additional treatment, including antibiotics or further surgical intervention.
Given that you have been experiencing this discharge for nearly four months, it is advisable to consult your healthcare provider for a thorough evaluation. They may perform a physical examination to assess the surgical site and determine if there are any signs of infection or other issues that need to be addressed. Additionally, they may recommend imaging studies or other diagnostic tests to rule out any underlying problems.
In the meantime, continuing with warm sitz baths can be beneficial as they help to keep the area clean and promote healing. Maintaining good hygiene is essential, and you should avoid any irritants that could exacerbate the situation. If you notice any changes in the discharge, such as increased volume, a change in color to green or brown, or if you develop fever, increased pain, or swelling, seek medical attention promptly.
In summary, while some discharge can be normal after anal fistula surgery, persistent or foul-smelling discharge warrants further investigation. It is crucial to communicate your concerns with your healthcare provider, who can provide tailored advice and treatment options based on your specific situation. Remember, recovery from surgery can take time, and being proactive about your health is key to ensuring a successful outcome.
