Buttock mass
Recently, I have noticed a lump on my buttock that is about 2 to 3 centimeters in size.
It causes pain when sitting or standing.
Could this be a cyst? What treatment is needed, and which specialist should I see?

Dr. Huang Ruiyun reply Dermatology
Hello: I recommend that you see a general surgeon or a plastic surgeon.
If the lesion is very close to the anus, a colorectal surgeon could also be an option.
Wishing you the best of luck.

More Info
When it comes to painful lumps in the buttock area, there are several potential causes, and understanding these can help guide you toward the appropriate treatment options. The lump you describe, measuring about 2-3 centimeters and causing pain when sitting or standing, could indeed be a cyst, but it may also represent other conditions such as a lipoma, abscess, or even a more serious issue like a tumor.
Possible Causes of Buttock Lumps
1. Cysts: These are fluid-filled sacs that can develop in various parts of the body, including the buttocks. A common type is a sebaceous cyst, which occurs when a hair follicle becomes blocked. Cysts are usually benign and may not require treatment unless they become infected or painful.
2. Lipomas: These are benign tumors made of fat tissue. They are generally soft to the touch and movable under the skin. Lipomas are typically painless but can cause discomfort if they press against nerves or other structures.
3. Abscesses: An abscess is a collection of pus that can occur due to infection. If the lump is red, warm, and painful, it may be an abscess that requires drainage and antibiotics.
4. Hematomas: If there has been any recent trauma to the area, a hematoma (a localized collection of blood outside of blood vessels) could form, leading to a painful lump.
5. Tumors: While less common, both benign and malignant tumors can present as lumps in the buttock area. If there are concerning features (such as rapid growth, hardness, or associated systemic symptoms like weight loss), further evaluation is warranted.
Treatment Options
The treatment for a painful lump in the buttock will depend on the underlying cause:
- Cysts: If the cyst is not infected and not causing significant discomfort, it may be monitored. If it becomes painful or infected, it may need to be drained or surgically removed.
- Lipomas: If the lipoma is asymptomatic, it may not require treatment. However, if it causes discomfort or cosmetic concerns, surgical removal is an option.
- Abscesses: These typically require drainage and may also need antibiotics to treat the underlying infection.
- Hematomas: Treatment may involve rest, ice, and observation. If the hematoma is large or painful, it may need to be drained.
- Tumors: If there is suspicion of a tumor, imaging studies (like an ultrasound or MRI) may be necessary, followed by a biopsy to determine the nature of the growth.
When to See a Doctor
Given the symptoms you describe, it is advisable to consult a healthcare provider for a proper evaluation. You may start with a visit to your primary care physician, who can assess the lump and determine if you need to see a specialist, such as a dermatologist (for skin-related issues) or a surgeon (for potential surgical intervention).
Conclusion
In summary, while the lump in your buttock could be a cyst, it is essential to have it evaluated by a healthcare professional to determine the exact cause and appropriate treatment. Early intervention can prevent complications and ensure that any necessary treatment is administered promptly. If you experience any additional symptoms, such as fever, increasing pain, or changes in bowel or urinary habits, seek medical attention immediately.
