White Particles in Urine During Bowel Movements - Internal Medicine

excuse me?

Hello Dr.
Yang: I would like to ask about the small white objects that appear when I urinate and have a bowel movement at the same time.
The size of these objects is about 0.5 cm or less.
I would like to know what they are.
When I only urinate, there are no white objects, but if I urinate and then have a bowel movement (especially when I exert some effort), white objects appear (this happens in 8 out of 10 times).
There is no pain or burning sensation during urination, and this situation has been ongoing for six months.
Previously, I had tests done with the following results: CHOL 152, HDL-C 52, CREA 0.8, TG 55, LDL-C 89.
What tests should I consider for further evaluation? Thank you.

Miss Ou, 30~39 year old female. Ask Date: 2010/11/18

Dr. Yang Weishun reply Internal Medicine

It is recommended that you perform a urine screening at least 5 to 7 days after your menstrual cycle ends to check for hematuria, proteinuria, or crystals.
If the urine test results are normal, you may consider a stool screening, as your description does not clarify whether the so-called white objects are from stool or urine.
Wishing you good health!

The presence of white particles in urine, especially when accompanied by bowel movements, can be concerning and warrants further investigation. Based on your description, it seems that these particles appear primarily when you urinate after having a bowel movement, and they do not cause any pain or discomfort. This is an important detail, as the absence of pain or burning sensations typically suggests that there may not be an acute infection or severe underlying condition.

White particles in urine can originate from several sources. They may be due to the presence of mucus, which is a normal secretion from the lining of the urinary tract. Mucus can sometimes appear as small white particles, especially if there is increased pressure during bowel movements that may cause some of this mucus to be expelled into the urine. Additionally, if you are experiencing any form of dehydration or changes in your diet, this could also affect the concentration and appearance of your urine.

Another possible explanation for the white particles could be related to the presence of crystals or sediment. Crystals can form in urine due to various factors, including dietary changes, dehydration, or certain medical conditions. However, since you mentioned that the particles do not appear when you urinate alone, it suggests that they may not be solely related to urinary crystals.

Given that you have been experiencing this issue for six months, it is advisable to consult with a healthcare professional for a thorough evaluation. A urinalysis would be a good starting point, as it can help identify the presence of any abnormal substances, such as proteins, glucose, or blood, which could indicate underlying health issues. Additionally, a urine culture may be necessary to rule out any infections that might not present with typical symptoms.

In terms of your previous health checks, your cholesterol levels and kidney function (as indicated by creatinine levels) appear to be within normal ranges. However, it is essential to consider other factors, such as hydration status, dietary habits, and any medications you may be taking, as these can all influence urine composition.

To summarize, while the presence of white particles in urine during bowel movements can be benign, it is crucial to investigate further, especially given the duration of your symptoms. A healthcare provider can guide you through the necessary tests and evaluations to determine the cause of these particles and ensure that your urinary and overall health is maintained. In the meantime, staying well-hydrated and monitoring any changes in your symptoms can be beneficial.
