Hematospermia issues
Hello Doctor, I first experienced hematospermia about three months ago, and after ejaculation with blood in the semen, my first urination would have hematuria and some blood clots.
At that time, you diagnosed it as a bacterial infection.
However, over the past three months, whenever I engage in slightly vigorous masturbation, I experience hematospermia again, followed by hematuria and blood clots during my first urination.
Today, I tried again and experienced hematospermia, but this time there were no blood clots in the hematuria, only something resembling a "blood sac." Aside from the symptoms mentioned, my other ejaculations that are not vigorous are white in color, and I don't feel any other abnormalities (such as in urination).
I kindly ask for your clarification, Doctor.

Hello: In response to your question, you can continue to observe.
If hematospermia persists, medication treatment may be required for 2 to 4 weeks.

More Info
Hematospermia, or the presence of blood in semen, can be a concerning symptom for many individuals. It is important to understand the potential causes, associated symptoms, and treatment options available.
Causes of Hematospermia
1. Infections: One of the most common causes of hematospermia is an infection in the reproductive system, such as prostatitis (inflammation of the prostate), epididymitis (inflammation of the epididymis), or urethritis (inflammation of the urethra). Bacterial infections can lead to inflammation and bleeding.
2. Trauma: Physical trauma to the genital area, whether from vigorous sexual activity, injury, or medical procedures, can result in blood in the semen.
3. Vascular Issues: Conditions that affect blood vessels, such as varicoceles (enlarged veins in the scrotum) or other vascular malformations, can lead to bleeding.
4. Benign Prostatic Hyperplasia (BPH): An enlarged prostate can cause bleeding, especially in older men.
5. Malignancies: Although less common, tumors in the prostate, seminal vesicles, or other parts of the reproductive system can cause hematospermia.
6. Systemic Conditions: Conditions such as hypertension or blood clotting disorders can also contribute to bleeding in the semen.
Symptoms Associated with Hematospermia
In addition to the presence of blood in semen, individuals may experience other symptoms such as:
- Blood in urine (hematuria)
- Pain or discomfort during ejaculation
- Pain in the lower abdomen or pelvic area
- Frequent urination or urgency
- Fever or chills (if an infection is present)
Diagnosis
To determine the underlying cause of hematospermia, a healthcare provider may conduct a thorough medical history and physical examination. Diagnostic tests may include:
- Urinalysis: To check for blood, infection, or other abnormalities in the urine.
- Semen analysis: To evaluate the quality and composition of the semen.
- Imaging studies: Such as ultrasound or MRI to visualize the reproductive organs.
- Cystoscopy: A procedure to look inside the bladder and urethra.
Treatment Options
The treatment for hematospermia depends on the underlying cause:
1. Antibiotics: If an infection is diagnosed, antibiotics will be prescribed to treat the infection.
2. Pain Management: Over-the-counter pain relievers may be recommended to alleviate discomfort.
3. Surgery: In cases where there is a structural issue, such as a varicocele or a tumor, surgical intervention may be necessary.
4. Observation: If the cause is benign and resolves on its own, a healthcare provider may recommend monitoring the condition.
5. Lifestyle Modifications: Reducing activities that may lead to trauma or irritation, such as vigorous sexual activity, may help prevent recurrence.
Conclusion
If you are experiencing recurrent episodes of hematospermia, especially with associated symptoms like blood in urine or the presence of clots, it is crucial to follow up with a healthcare provider for further evaluation. While many cases of hematospermia are benign and resolve on their own, persistent or severe symptoms warrant a thorough investigation to rule out serious underlying conditions. Early diagnosis and appropriate treatment can help manage symptoms effectively and address any potential health concerns.
