Gynecomastia

My male friend previously had gynecomastia and underwent surgery.
However, after the surgery, the area around the areola feels hard to the touch (before the surgery, pressing the nipple would reveal a hard lump; after the surgery, the surface of the areola feels hard).
What should be done about this?

Mao Mao, 20~29 year old female. Ask Date: 2011/03/12

Dr. Wu Jingtang reply Breast and Thyroid

Dear Mao Mao,
It is possible that after surgery, some tissue may remain beneath the nipple-areola complex, and the internal wounds may lead to the formation of scar tissue or the accumulation of seroma.
It is recommended to return to the hospital's breast surgery outpatient clinic for a breast ultrasound to further confirm the diagnosis.
Wishing you safety and health,

Gynecomastia, or the enlargement of breast tissue in males, can be a source of significant psychological distress and physical discomfort. Surgical intervention, often in the form of a mastectomy or liposuction, is a common treatment for this condition. However, post-operative concerns can arise, including changes in the texture and firmness of the breast tissue, particularly around the nipple area.

In your friend's case, the hardness felt around the areola (the pigmented area surrounding the nipple) after surgery could be attributed to several factors. One common reason for this post-surgical hardness is the formation of scar tissue. After any surgical procedure, the body initiates a healing response, which includes the formation of scar tissue. This tissue can sometimes feel firmer than the surrounding areas, leading to the sensation of hardness.
Another possibility is the presence of residual glandular tissue. In some cases, if the gynecomastia was particularly pronounced, not all of the glandular tissue may be removed during surgery, which could lead to a firm feeling in the area. Additionally, swelling and inflammation post-surgery can contribute to the firmness felt around the nipple.

To address these concerns, here are some recommendations:
1. Follow-Up with the Surgeon: It's essential for your friend to have a follow-up appointment with the surgeon who performed the procedure. The surgeon can assess the area, determine if the hardness is due to scar tissue or residual tissue, and provide guidance on the next steps.

2. Massage Therapy: Gentle massage of the area may help in breaking down scar tissue and improving the texture of the skin. However, this should be done cautiously and ideally under the guidance of a healthcare professional.

3. Compression Garments: Wearing a compression garment as recommended by the surgeon can help reduce swelling and support the healing process. This can also help in shaping the chest area post-surgery.

4. Physical Therapy: If the hardness persists and is bothersome, a referral to a physical therapist who specializes in post-surgical recovery may be beneficial. They can provide targeted exercises and treatments to help alleviate discomfort and improve the overall appearance.

5. Patience and Time: Healing from surgery takes time. It is not uncommon for the firmness to gradually decrease over several months as the body continues to heal and remodel the tissue.

6. Monitor for Other Symptoms: If your friend experiences any unusual symptoms, such as increased pain, redness, or discharge from the nipple, he should seek medical attention promptly, as these could indicate complications such as infection or hematoma.

In conclusion, while post-surgical hardness around the areola can be concerning, it is often a normal part of the healing process. With appropriate follow-up care and management strategies, your friend can work towards achieving the desired outcome and alleviating any discomfort. It’s crucial to maintain open communication with healthcare providers throughout the recovery process to ensure the best possible results.
