Frequent urination issues?
Hello Doctor: About two weeks ago, I suddenly felt a frequent urge to urinate.
After using the restroom, I still felt the urge (but there is no pain or blood in the urine), and I experienced a tight, uncomfortable sensation at the tip of the penis.
After visiting a urologist, the doctor initially treated me for acute cystitis/urethritis during the first week, but after taking medication for a week, there was no significant improvement.
In the second week, the treatment was adjusted to mild overactive bladder, but after another week of medication, I still feel limited improvement.
The doctor then speculated that it might be chronic cystitis.
I would like to ask the following questions: (1) What could be the possible causes? Besides bladder issues, could it be related to the foreskin or glans? (2) Recently, I have noticed a delay of a few seconds before I can urinate, and after urination, there is a slight discomfort in the penis.
Is this related to my condition? (3) How long does the typical treatment course usually take? Thank you!

Dr. Xu Weikai reply Urology
Xiao Cai: Hello, it is still more likely to be a bladder infection.
Additionally, it is possible that even after the bacteria have been eradicated, the damage caused by the bladder infection has not yet healed.
Therefore, it requires time for repair.
If frequent urination is severe, using a warm towel or heating pad on the bladder area below the navel should be effective.
Furthermore, applying a warm towel to the penis can improve circulation, which may also promote tissue repair.
If symptoms persist for more than a month, it may be worth considering whether it is caused by urinary stones, and an ultrasound examination along with relevant X-rays can be performed.
The entire treatment course varies from person to person.
Some people recover in three days, while others may take several months or even years to see improvement.
Of course, it is essential to identify the underlying cause of the condition.

More Info
Frequent urination can be a distressing symptom that may arise from various underlying conditions. Understanding the potential causes and treatment options is essential for effective management. Based on your description, it seems you have already consulted with healthcare professionals, but let’s delve deeper into your concerns.
(1) Possible Causes of Frequent Urination
Frequent urination can be attributed to several factors, including:
- Infections: Urinary tract infections (UTIs) are a common cause of increased urinary frequency. Although you mentioned that you were treated for acute cystitis and did not see significant improvement, it’s important to rule out any persistent or recurrent infections.
- Bladder Overactivity: This condition, also known as overactive bladder (OAB), can lead to a sudden urge to urinate frequently. It may not always be associated with pain or discomfort.
- Chronic Cystitis: If the inflammation of the bladder persists, it can lead to chronic cystitis, which may cause ongoing symptoms similar to those of a UTI.
- Urethral Issues: Conditions affecting the urethra, such as inflammation or strictures, can also contribute to urinary symptoms. Given your mention of discomfort at the tip of the penis, it’s worth considering whether there might be an issue related to the foreskin or glans, such as balanitis or posthitis.
- Psychogenic Factors: Anxiety and stress can exacerbate urinary symptoms, leading to increased frequency and urgency.
- Neurological Conditions: Conditions affecting the nervous system can disrupt the normal function of the bladder and urinary tract.
(2) Connection Between Symptoms
The sensation of a delay in urination and the subsequent discomfort you feel could indeed be related to your underlying condition. The feeling of tightness or soreness at the tip of the penis may indicate irritation or inflammation, which could be linked to:
- Urethritis: Inflammation of the urethra can cause discomfort and may be associated with frequent urination.
- Bladder or Urethral Spasms: These spasms can lead to a sensation of urgency and may cause a delay in the initiation of urination.
- Psychological Factors: Anxiety about urination can lead to muscle tension, which may contribute to the sensation of delayed urination.
(3) Treatment Duration and Options
The duration of treatment for conditions like overactive bladder or chronic cystitis can vary widely. Generally, treatment may last from several weeks to months, depending on the underlying cause and response to therapy. Here are some common treatment approaches:
- Medications: Anticholinergic medications are often prescribed for overactive bladder. If you have not seen improvement, your doctor may consider adjusting the dosage or trying a different medication.
- Behavioral Therapies: Bladder training and pelvic floor exercises can help manage symptoms effectively.
- Dietary Modifications: Avoiding irritants such as caffeine, alcohol, and spicy foods may help reduce symptoms.
- Follow-Up Testing: If symptoms persist, further evaluation may be necessary, including urine cultures, imaging studies, or cystoscopy to visualize the bladder and urethra.
- Referral to Specialists: If initial treatments are ineffective, referral to a urologist or a specialist in pelvic floor disorders may be warranted.
Conclusion
In summary, frequent urination can stem from a variety of causes, and understanding these can help guide appropriate treatment. It is essential to maintain open communication with your healthcare provider about your symptoms and any changes you experience. If your current treatment plan is not yielding results, do not hesitate to seek further evaluation or a second opinion. Your comfort and health are paramount, and there are multiple avenues to explore for relief.
