Pain during ejaculation?
During ejaculation, I experience a cramping pain in the last few thrusts (initially, there is no pain during ejaculation).
This has been ongoing for about a month now (the cramping sensation has not intensified), and I do not have any noticeable pain or abnormalities in my penis or testicles.
Thank you, doctor, for your guidance.

Hello, general ejaculation pain may be caused by the following conditions: 1.
Prolonged erection or excessive intensity during the process, leading to localized soreness.
2.
Autonomic nervous system dysfunction.
3.
Local tumors or fibrosis.
After ejaculation, applying a warm towel for local heat therapy may help improve blood circulation in the area.
However, if the pain persists, there may be other underlying issues that require further examination and treatment.
If there are any unclear points, feel free to ask again for further discussion and research.

Post-ejaculation pain, also known as post-orgasmic illness syndrome (POIS), is a condition that can cause discomfort or pain after ejaculation. This phenomenon can be perplexing and concerning for those who experience it, especially if it persists for an extended period, as in your case, lasting about a month. Understanding the potential causes and solutions is essential for addressing this issue effectively.
Possible Causes of Post-Ejaculation Pain
1. Muscle Tension and Spasms: One of the most common causes of post-ejaculation pain is muscle tension or spasms in the pelvic region. During sexual arousal and ejaculation, various muscles contract, and if these muscles remain tense or are overworked, it can lead to pain or discomfort afterward.
2. Prostatitis: Inflammation of the prostate gland, known as prostatitis, can cause pain during or after ejaculation. This condition can be acute or chronic and may be accompanied by other symptoms such as difficulty urinating, pelvic pain, or flu-like symptoms.
3. Epididymitis: This is an inflammation of the epididymis, the tube at the back of the testicle that stores and carries sperm. Epididymitis can cause pain in the testicles and may be exacerbated by ejaculation.
4. Hormonal Changes: Hormonal fluctuations can also play a role in post-ejaculation pain. Changes in testosterone levels or other hormones can affect how the body responds during and after sexual activity.
5. Infections: Sexually transmitted infections (STIs) or urinary tract infections (UTIs) can lead to pain during or after ejaculation. If there is an underlying infection, it is crucial to seek medical attention.
6. Psychological Factors: Anxiety or stress related to sexual performance or other life factors can manifest as physical symptoms, including pain during or after ejaculation.
Solutions and Recommendations
1. Medical Evaluation: It is essential to consult a healthcare provider, preferably a urologist, to discuss your symptoms in detail. They may perform a physical examination, order blood tests, or conduct imaging studies to rule out any underlying conditions such as prostatitis or infections.
2. Pelvic Floor Exercises: Engaging in pelvic floor exercises, often referred to as Kegel exercises, may help strengthen the pelvic muscles and alleviate tension. These exercises can improve muscle control and potentially reduce pain.
3. Relaxation Techniques: Stress management techniques such as deep breathing, meditation, or yoga can help reduce overall muscle tension and anxiety, which may contribute to post-ejaculation pain.
4. Medication: Depending on the underlying cause, your doctor may prescribe anti-inflammatory medications, muscle relaxants, or antibiotics if an infection is present. Over-the-counter pain relievers may also provide temporary relief.
5. Avoiding Irritants: If you suspect that certain activities or substances (like alcohol or certain foods) exacerbate your symptoms, consider avoiding them to see if your condition improves.
6. Follow-Up: If symptoms persist despite initial treatment, follow up with your healthcare provider. They may recommend further evaluation or refer you to a specialist for more targeted treatment.
In conclusion, post-ejaculation pain can arise from various causes, ranging from muscle tension to infections. It is crucial to seek medical advice to identify the underlying issue and receive appropriate treatment. By addressing both the physical and psychological aspects of this condition, you can work towards finding relief and improving your overall sexual health.
