White pain?
Doctor, I have two questions.
First: I experience a stinging sensation when I use the restroom, and my genital area itches, especially at night.
About a year or two ago, I had an injury from a bicycle seat and I'm concerned it might be gonorrhea, but I’m afraid to tell my parents.
Are there any home remedies? Second: I have some small white spots at the junction of my penis and foreskin.
They hurt when I squeeze them and are irregularly arranged.
Would circumcision help?
Guo Yi Sheng, 10~19 year old female. Ask Date: 2011/05/07
Dr. Liu Jiaju reply Urology
Gonorrhea is primarily transmitted through sexual intercourse.
It is unlikely to contract gonorrhea from coming into contact with a bicycle seat.
It is advisable to seek medical attention for a thorough evaluation rather than relying on any home remedies.
The white substance in the penile coronal sulcus may be smegma or benign pearly papules.
Whether circumcision is necessary or if it would improve the condition should also be assessed by a physician.
Thank you!

More Info
When it comes to urological concerns such as painful urination and the presence of white bumps, it is essential to approach the situation with a clear understanding of potential underlying issues. Your symptoms, including painful urination, itching in the genital area, and the presence of white bumps, could indicate several conditions that warrant further investigation.
Painful Urination and Itching
The pain you experience during urination, especially if it is accompanied by itching, could be indicative of a urinary tract infection (UTI), sexually transmitted infections (STIs) such as gonorrhea or chlamydia, or even irritation from soaps or hygiene products. Given your history of trauma to the genital area and your concerns about potential STIs, it is crucial to seek medical attention for a proper diagnosis.
White Bumps and Discomfort
The white bumps you describe at the junction of the penis and foreskin could be several things. They might be pearly penile papules, which are harmless and common anatomical variations. However, they could also be indicative of a condition such as balanitis, which is inflammation of the glans (the head of the penis), or a sexually transmitted infection that could cause lesions. The pain associated with these bumps, especially when pressure is applied, suggests that there may be an underlying infection or inflammation.
Recommendations
1. Seek Medical Attention: It is vital to consult a healthcare professional, preferably a urologist, who can perform a thorough examination and possibly run tests such as urinalysis and swabs to check for infections. This is particularly important given your concerns about STIs.
2. Avoid Self-Diagnosis and Treatment: While it may be tempting to look for home remedies or "folk cures," these can often do more harm than good. Conditions like STIs require specific treatments that only a healthcare provider can prescribe.
3. Hygiene Practices: Maintain good genital hygiene, but avoid using harsh soaps or products that could irritate the area. Gentle cleansing with water is often sufficient.
4. Discuss Concerns Openly: If you are worried about discussing your symptoms with your parents, consider speaking to a trusted adult or a healthcare provider directly. They can provide you with the support and guidance you need.
5. Consider Circumcision: If the white bumps are determined to be a result of recurrent infections or irritation, circumcision may be suggested as a treatment option. However, this should be discussed thoroughly with your healthcare provider, who can explain the benefits and risks involved.
Conclusion
In summary, your symptoms of painful urination, itching, and the presence of white bumps are concerning and should not be ignored. Seeking professional medical advice is the best course of action to ensure proper diagnosis and treatment. Early intervention can prevent complications and lead to better health outcomes. Remember, your health is important, and addressing these issues promptly is crucial.
