Erection and ejaculation

Hello, Dr.
Lin.
I would like to ask you three questions.
Here they are:
1.
Sometimes, I experience a sudden sensation of ejaculation, and my penis becomes partially erect, even without any sexual stimulation.
This sensation can last for 3 to 5 hours.
If I touch my penis during this time (not for masturbation), it becomes fully erect, and the sensation of ejaculation intensifies, with pre-ejaculatory fluid being released from the glans.
What could be the reason for this? Does this situation indicate that ejaculation needs to be released?
2.
Why does the sensation of ejaculation often go unspoken? What is the mechanism of this process?
3.
Occasionally, when I hold my urine for a longer time, the sensation of relief feels similar to ejaculation.
During urination, I also experience an erection and notice a lot of bubbles.
Does this contain protein (like semen)?
Thank you.

Hello, sir: Thank you for your question.
I would like to respond to your inquiry and provide the following suggestions: Due to age, blood circulation tends to be better, making it easier to achieve an erection.
The clear mucus that is released during the pre-erection phase does not necessarily indicate that ejaculation is imminent; its primary function is to provide lubrication during sexual activity.
The sensation of ejaculation, referred to as orgasm, varies from person to person and cannot be precisely described.
The process of ejaculation involves the sympathetic nervous system working in conjunction with the somatic nervous system, leading to intense contractions of the bulbospongiosus and ischiocavernosus muscles, along with the contraction of the bladder, which expels semen from the body.
This is a complex physiological process.
Holding urine for an extended period or having a full bladder can more easily lead to an erection.
This phenomenon is a response triggered by parasympathetic stimulation.
When one can hold urine for a long time, it is often due to insufficient water intake, which increases the likelihood of this occurrence.
When hydration is low, urine becomes more concentrated, leading to the formation of bubbles during urination, which is not ejaculation.
If there is protein present, bubbles may appear every time one urinates.
Lastly, I wish you peace and happiness!

Understanding unexplained ejaculation sensations and related issues can be complex and multifaceted. Let's break down your questions one by one, considering both physiological and psychological aspects.

1. Sudden Ejaculation Sensations and Semi-Erection: The sensations you describe, where you experience a sudden feeling of ejaculation without any sexual stimulation, can be attributed to several factors. The phenomenon of experiencing a semi-erection and the sensation of ejaculation can be linked to the autonomic nervous system, which controls involuntary bodily functions, including sexual arousal and ejaculation. The body can sometimes respond to non-sexual stimuli or even internal thoughts and feelings, leading to these sensations.
The presence of pre-ejaculatory fluid (also known as pre-cum) during this state is normal. This fluid is produced by the Cowper's glands and serves to lubricate the urethra and neutralize acidity in the urethra prior to ejaculation. The fact that you can achieve a full erection with light stimulation indicates that your erectile function is intact, but the sensations you're experiencing may not necessarily lead to ejaculation unless there is sufficient stimulation.

2. Transmission of Ejaculation Sensation: The sensation of ejaculation is primarily mediated by the nervous system. The pelvic nerves play a crucial role in transmitting signals from the genital area to the brain. When the body is aroused, signals are sent through these nerves, leading to the feelings associated with ejaculation. This process can occur even in the absence of direct sexual stimulation due to the brain's ability to interpret various stimuli, including emotional and psychological factors.
Additionally, the brain's limbic system, which is involved in emotional responses, can trigger these sensations based on memories, fantasies, or even stress. This means that the experience of ejaculation can sometimes be more about the brain's interpretation of bodily signals rather than a direct physical stimulus.

3. Urination and Associated Sensations: The sensations you experience when holding urine for an extended period can indeed mimic those of ejaculation. This is because the bladder and the reproductive system share some of the same nerve pathways. When the bladder is full, it sends signals to the brain that can be interpreted similarly to sexual arousal. The pressure and discomfort from a full bladder can lead to involuntary contractions of the pelvic muscles, which may also cause a sensation similar to ejaculation.

The presence of bubbles in your urine can be attributed to several factors, including the force of urination and the presence of proteins or other substances in the urine. While normal urine can have bubbles due to the physical dynamics of urination, persistent foamy urine may indicate proteinuria, which could be a sign of kidney issues. If this is a recurring concern, it would be advisable to consult a healthcare professional for further evaluation.

In summary, the sensations you're experiencing can be linked to the complex interplay between the nervous system, psychological factors, and physiological responses. If these sensations are causing you distress or if you have concerns about your sexual health or urinary function, it would be prudent to consult a urologist or a healthcare provider specializing in sexual health. They can provide a thorough evaluation and address any underlying issues that may be contributing to your symptoms.
