Treatment of erectile dysfunction?
Dear Dr.
Hsu,
I have some questions regarding erectile dysfunction.
I have been diagnosed with diabetes for nearly ten years, and during this time, my blood sugar control has been poor at times.
Since 2006, I suddenly experienced erectile dysfunction, with a complete inability to achieve an erection that lasted for several years.
In 2009, I started dating a new girlfriend, and during our sexual activities, I was still unable to achieve an erection.
Even when I managed to ejaculate, I felt that the semen was entering the bladder.
We have plans to start a family, so I hope to seek treatment to have children.
However, I have not seen any results from treatments in the United States.
I have tried oral medications such as Viagra and Cialis, as well as injection therapy, but none have been effective.
I would like to ask Dr.
Hsu:
1.
Is there a chance for a cure for this erectile dysfunction?
2.
If I consider surgical treatment, are there any potential side effects? How much would such surgery cost out of pocket, and how long would the entire treatment process take?
Thank you, Dr.
Hsu.
I hope to receive your response soon.

Dr. Xu Weikai reply Urology
Robert: Hello, the question you raised cannot be answered without a personal examination.
A diagnosis can only be made after determining the underlying cause.
Different types of sexual dysfunction have various treatment options and surgical approaches.
A thorough evaluation is necessary for accurate diagnosis and treatment.
If there are any unclear points, feel free to ask again, and we can discuss and research further.
This response is from Dr.
Hsu Wei-Kai, Director of the Urology Department, Taitung Hospital, Department of Health, Executive Yuan.

More Info
Erectile dysfunction (ED) is a common complication among diabetic patients, particularly those with a long history of poorly controlled blood sugar levels. The relationship between diabetes and ED is multifaceted, involving both physiological and psychological factors. Given your situation, let’s explore the potential causes, treatment options, and the prospects for recovery.
Understanding the Causes
1. Neuropathy: Diabetes can lead to nerve damage (diabetic neuropathy), which affects the nerves responsible for erections. This can result in reduced sensation and impaired blood flow to the penis.
2. Vascular Issues: Diabetes is associated with vascular complications, including atherosclerosis (hardening of the arteries), which can impede blood flow necessary for achieving and maintaining an erection.
3. Hormonal Factors: Diabetes can also affect hormone levels, including testosterone, which plays a crucial role in sexual function.
4. Psychological Factors: The stress and anxiety associated with diabetes and its complications can further exacerbate ED. Relationship issues, performance anxiety, and depression are common psychological contributors.
Treatment Options
1. Oral Medications: You mentioned trying oral medications like Viagra (sildenafil) and Cialis (tadalafil) without success. While these medications are effective for many, they may not work for everyone, especially if the underlying causes are severe.
2. Injection Therapy: You also tried injection therapy, which involves injecting medication directly into the penis to induce an erection. If this has not been effective, it may be worth discussing alternative medications or dosages with your healthcare provider.
3. Vacuum Erection Devices (VED): These devices create a vacuum around the penis, drawing blood into it and causing an erection. They are non-invasive and can be effective for many men.
4. Penile Implants: If other treatments fail, surgical options such as penile implants may be considered. This involves placing a device inside the penis that allows for an erection.
Surgical Considerations
1. Effectiveness: Penile implants have a high satisfaction rate among patients and their partners. They can provide a permanent solution to ED, especially when other treatments have failed.
2. Risks and Side Effects: As with any surgery, there are risks involved, including infection, device malfunction, and potential changes in sensation. It’s crucial to discuss these risks with your surgeon.
3. Cost and Duration: The cost of penile implants can vary widely based on the type of device and the healthcare facility. In the U.S., the total cost can range from $10,000 to $30,000, including the procedure and follow-up care. The recovery time is typically a few weeks, but you should consult with your surgeon for a personalized timeline.
Prognosis and Future Steps
1. Potential for Recovery: While complete "cure" of ED may not be possible for everyone, many men experience significant improvement with the right treatment. It’s important to maintain good blood sugar control, as this can help prevent further complications.
2. Lifestyle Modifications: In addition to medical treatments, lifestyle changes such as regular exercise, a healthy diet, and quitting smoking can improve overall vascular health and potentially enhance erectile function.
3. Psychological Support: Consider seeking counseling or therapy to address any psychological factors contributing to your ED. This can be particularly beneficial if anxiety or depression is present.
4. Regular Follow-Up: Continuous follow-up with your healthcare provider is essential to monitor your diabetes and adjust treatment plans as necessary.
In conclusion, while your situation is challenging, there are multiple avenues to explore for treating erectile dysfunction related to diabetes. It’s crucial to have open discussions with your healthcare team about your concerns and treatment options. With the right approach, many men can regain their sexual function and improve their quality of life.
