Nail injury and deformity issues?

A few years ago, I injured a finger near the nail, and since then, the nail that grew back has developed a straight line, causing it to be uneven with the left side higher than the right.
A dermatologist mentioned that this was due to damage to the nail matrix.
Is there a surgical option to restore the nail to its normal shape? Which specialty should I consult for this issue? Thank you!

Niming, 20~29 year old female. Ask Date: 2011/09/26

Dr. Guo Fengsheng reply Dermatology

This condition cannot be restored through surgery, and new surgical procedures may cause additional harm; the appearance of the nails may not necessarily improve.

Nail deformities can indeed be corrected, but the treatment options and their effectiveness depend on the specific nature of the deformity and the underlying cause. In your case, it sounds like the injury you sustained several years ago affected the nail matrix, which is the tissue under the base of the nail that produces new nail cells. Damage to this area can lead to various nail deformities, including ridges, splits, or uneven growth, as you've described with your nail being higher on one side than the other.

Understanding Nail Deformities
Nail deformities can arise from a variety of factors, including trauma (like your injury), infections, systemic diseases, or congenital conditions. When the nail matrix is damaged, it can disrupt the normal growth pattern of the nail, leading to permanent changes in its appearance. In your case, the straight line and uneven growth suggest that the nail matrix was affected, which can result in a nail that does not grow back to its original shape.

Treatment Options
1. Observation: In some cases, if the deformity is not causing pain or functional issues, doctors may recommend simply monitoring the nail over time. Nails grow slowly, and it may take several months for a new nail to fully grow out.

2. Nail Surgery: If the deformity is significant and causing discomfort or functional issues, surgical intervention may be necessary. This could involve procedures such as:
- Nail Matrix Surgery: If the nail matrix is severely damaged, a surgeon may need to remove the affected portion of the nail matrix to allow for proper nail growth.

- Nail Plate Removal: In some cases, the nail plate may need to be removed temporarily to allow for treatment of the underlying issue.

- Corrective Surgery: In more severe cases, reconstructive surgery may be performed to reshape the nail bed or matrix.

3. Dermatological Treatments: Consulting a dermatologist is a good first step. They can assess the condition of your nail and recommend appropriate treatments, which may include topical treatments or procedures to stimulate healthy nail growth.

4. Podiatrist Consultation: If the nail deformity is on a toe, a podiatrist (foot specialist) may be the appropriate specialist to consult. They can provide treatment options specific to toenail issues.

5. Nail Care: In addition to medical treatments, proper nail care can help manage the appearance of the nail. This includes keeping the nail trimmed, avoiding trauma, and using protective coverings if necessary.

Follow-Up and Prognosis
The prognosis for correcting nail deformities largely depends on the extent of the damage to the nail matrix and the type of treatment pursued. If the nail matrix is still functional and healthy, there is a good chance that with the right treatment, the nail can grow back in a more normal shape. However, if the matrix is severely damaged, the nail may continue to grow abnormally.

Conclusion
In summary, it is advisable to consult a dermatologist or a podiatrist to evaluate your specific situation. They can provide a thorough examination and recommend the best course of action based on the severity of the deformity and any associated symptoms. Early intervention may lead to better outcomes, so seeking professional advice is a crucial step in addressing nail deformities effectively.
