Cauliflower-like and pearl-like papules?

Hello Doctor: Two years ago, I had genital warts, and after treatment with electrocautery, there has been no recurrence.
However, I recently noticed a whole row of bumps on my glans, which are in a different location from where the warts initially appeared.
I have been reading some articles and I can't distinguish whether these are warts or pearly penile papules.
The current bumps feel very similar to the latter, but considering my history of warts, I'm unsure what to do.
I will be going to work in mainland China for three months in a few days, and I've been busy preparing, so I can't find the time to see a doctor.
I would like to ask you whether this is a recurrence of genital warts or if I have developed pearly penile papules.

Xiao Xiao, 20~29 year old female. Ask Date: 2011/09/29

Dr. Xu Weikai reply Urology

Hello, you can visit a nearby dermatology or urology clinic for an examination to confirm the diagnosis.
Only after a precise diagnosis can treatment be initiated.

Understanding the difference between warts (specifically genital warts caused by the human papillomavirus, or HPV) and pearly penile papules (PPP) is essential for anyone experiencing changes in the genital area.
Genital warts are typically soft, fleshy growths that can appear on the genital area, including the penis, vulva, and around the anus. They are caused by certain strains of HPV, which is a sexually transmitted infection. Warts can vary in size and may appear as single or multiple lesions. They can be itchy or uncomfortable, and in some cases, they may bleed. The fact that you had a previous diagnosis of genital warts and underwent treatment (such as electrosurgery) does raise the concern of a potential recurrence, especially if you have noticed new growths.

On the other hand, pearly penile papules are small, dome-shaped bumps that typically appear in a single row around the corona of the glans (the head of the penis). They are a normal anatomical variation and are not caused by an infection. PPP is harmless and does not require treatment. These papules are often mistaken for warts due to their appearance, but they are not associated with HPV and do not pose any health risks.

Given your history of genital warts, it is understandable to be concerned about the new growths you have noticed. However, there are some distinguishing features that can help you differentiate between the two:
1. Location: Genital warts can appear anywhere in the genital region, while pearly penile papules are specifically located around the glans of the penis.

2. Appearance: Genital warts tend to be irregular in shape and may have a cauliflower-like appearance. In contrast, pearly penile papules are uniform, smooth, and typically appear in a single row.

3. Symptoms: Genital warts may cause discomfort, itching, or bleeding, while pearly penile papules are usually asymptomatic and do not cause any pain or discomfort.

4. History of HPV: Since you have a history of genital warts, it is important to monitor any new growths closely. If they resemble your previous warts in appearance or if you experience any symptoms, it would be prudent to seek medical advice as soon as possible.

While it is understandable that you may be busy preparing for your upcoming work trip, it is crucial to prioritize your health. If you are unable to see a healthcare provider before you leave, consider scheduling an appointment as soon as you return. In the meantime, avoid any sexual activity that could potentially transmit HPV to others until you have a clearer understanding of your condition.

In summary, while pearly penile papules are harmless and do not require treatment, genital warts can indicate an active HPV infection that may need to be addressed. If you are uncertain about the nature of the growths you are experiencing, it is always best to consult with a healthcare professional for an accurate diagnosis and appropriate management. Your health and peace of mind are worth the effort.
