Retracting during urination and experiencing fresh blood after ejaculation?
Hello Dr.
Zhou,
I have two questions to ask:
1.
When I need to urinate, I have to wait about six seconds.
Just as I am about to urinate, it retracts automatically, and I have to wait a few more seconds.
This retraction happens three times in a row before I can successfully urinate.
This issue has been ongoing for almost a year.
2.
Six months ago, while I was waiting at a red light, my car was hit from behind by another vehicle traveling at 40 km/h.
My car was pushed forward, and the rear bumper was slightly dented.
Three days after the incident, I experienced bleeding after ejaculation, which made me concerned that it might be related to the accident.
I went to the hospital to check for blood in my urine, and the results showed that my urine was normal and did not contain blood.
For your reference, the automatic retraction when I need to urinate occurred before the accident, but the bleeding after ejaculation happened three days after the accident.
I have never experienced this before, and it only happened that one time; I have not had any bleeding after ejaculation since then.
Could you please help me understand this situation? Thank you.

Hello: Regarding the issue of hematuria and blood in the semen, further detailed examinations are necessary to clarify the cause.
It is recommended that you visit a urology clinic for further evaluation.
Thank you!

Hello, thank you for reaching out with your concerns regarding urinary issues and post-ejaculation bleeding. Let's break down your questions and address them one by one.
1. Urinary Hesitation and Retraction: You mentioned experiencing a delay of about six seconds before urination, along with a sensation of the urinary stream retracting multiple times before you can successfully urinate. This condition could be indicative of a few potential issues. One possibility is a condition known as detrusor instability, where the bladder muscle (detrusor) does not contract properly, leading to difficulty initiating urination. Another possibility is bladder outlet obstruction, which can occur due to various reasons, including prostate enlargement in men or other anatomical issues. It's also worth considering that anxiety or stress can contribute to urinary hesitancy. Given that this has been ongoing for nearly a year, I recommend consulting a urologist for a thorough evaluation, which may include a physical examination, urinalysis, and possibly urodynamic studies to assess bladder function.
2. Post-Ejaculation Bleeding: You reported experiencing bleeding after ejaculation three days following a car accident. While the urine tests showed no signs of blood, the timing of your symptoms raises some important considerations. Post-ejaculation bleeding can occur due to several reasons, including trauma to the genital area, infections, or even benign conditions like prostatitis or urethritis. Given that you were involved in a car accident, it's plausible that the impact could have caused some minor trauma to the pelvic region, potentially leading to the bleeding you experienced. However, since this incident occurred only once and has not recurred, it may not be directly related to the accident.
It's important to note that while the initial bleeding may have been alarming, the absence of further episodes and normal urinalysis is reassuring. However, if you experience any recurrence of bleeding, pain, or other concerning symptoms, it would be wise to seek medical attention promptly.
In summary, both of your concerns warrant further investigation by a healthcare professional. For the urinary issues, a urologist can provide a comprehensive assessment and recommend appropriate treatment options. For the post-ejaculation bleeding, while it seems to have resolved, keep an eye on any future symptoms and consult a doctor if they arise again.
Lastly, it's crucial to maintain open communication with your healthcare provider about any changes in your symptoms or concerns you may have, as this will help ensure you receive the best possible care. If you have any more questions or need further clarification, feel free to ask. Take care!
