Abdominal pain, fever, and rectal bleeding?

Hello Doctor: I have been experiencing many gastrointestinal issues this year.
First, I had constipation, and a traditional Chinese medicine practitioner said it was due to blood deficiency.
Whenever I drink a specific herbal soup, my bowel movements return to normal.
However, after a while, I drank two cups of homemade milk one day and started experiencing abdominal pain (it feels like it's above my belly button).
Following that, I had diarrhea, initially passing dark-colored stools, which then became loose and watery.
Now, my stools are consistently watery; every time I eat, I end up with watery stools.
I feel very fatigued in the mornings and often want to sleep.
Yesterday afternoon, I experienced a fever, but after exercising vigorously, it subsided.
Last night, after dinner, I felt the urge to go to the bathroom, but I only passed a small amount, and surprisingly, there was a drop of blood.
My anus does not hurt, and the watery stool and blood were separate.
Doctor, could this be enteritis? I immediately went to a nearby hospital, and the doctor prescribed me Buscopan and Gasgel.
Upon checking, I found out that one is for gastric relief and the other is for duodenal issues.
Doctor, is this phenomenon indicative of enteritis? Should I undergo more detailed examinations?

Hello: In fact, the condition can be divided into three parts, which are unrelated and all represent acute illnesses.
The first is constipation, possibly caused by poor dietary habits and bowel movements at that time.
The second is enteritis, which may be due to consuming unpasteurized milk.
The third is hematochezia, which is essentially bleeding from the anus or rectum.
Considering the age, the likelihood of internal hemorrhoids is the highest.
If there is significant bleeding or frequent occurrences, or if there are concerns, a consultation with a colorectal surgeon can be arranged.
Wishing you good health, Dr.

More Info

Abdominal pain, fever, and the presence of blood in stool can be concerning symptoms that warrant careful evaluation. Based on your description, it seems that you have experienced a series of gastrointestinal issues, including constipation, diarrhea, and now the presence of blood in your stool. Let's break down these symptoms and their potential implications from a gastroenterological perspective.

Understanding Your Symptoms
1. Abdominal Pain: The location of your abdominal pain, particularly in the area above the navel, can indicate various gastrointestinal issues. Pain in this region can be associated with conditions affecting the stomach, small intestine, or even the pancreas. Given that you have experienced pain after consuming certain foods, it may suggest a food intolerance or an underlying gastrointestinal condition.

2. Diarrhea: The transition from dark-colored stools to watery diarrhea is significant. Dark stools can indicate the presence of digested blood, which may suggest bleeding higher up in the gastrointestinal tract, such as from a gastric ulcer or esophageal varices. Watery diarrhea, especially following abdominal pain, can be indicative of an infectious process, such as viral or bacterial gastroenteritis.

3. Fever: The presence of fever alongside gastrointestinal symptoms often points to an infectious etiology. Infections can lead to inflammation of the gastrointestinal tract, resulting in symptoms like diarrhea and abdominal pain.

4. Blood in Stool: The appearance of blood in your stool, even if it is separate from the watery diarrhea, is a concerning symptom. It could indicate several conditions, including hemorrhoids, anal fissures, or more serious conditions like inflammatory bowel disease (IBD) or colorectal cancer, especially given your family history of cancer.

Possible Diagnosis
Based on your symptoms, it is plausible that you may be experiencing a form of gastroenteritis, which could be viral or bacterial in nature. However, the presence of blood in your stool necessitates further investigation. Conditions such as ulcerative colitis or Crohn's disease (types of IBD) could also present with similar symptoms, including abdominal pain, diarrhea, and blood in the stool.

Recommended Actions
1. Medical Evaluation: It is crucial to follow up with a healthcare provider for a thorough evaluation. Given your symptoms, a gastroenterologist may recommend further diagnostic tests, such as a colonoscopy or imaging studies, to assess the condition of your gastrointestinal tract.

2. Stool Tests: A stool sample may be analyzed to check for pathogens, blood, and other abnormalities. This can help identify if an infection is present and guide treatment.

3. Dietary Considerations: Until you receive a definitive diagnosis, it may be wise to maintain a bland diet, avoiding dairy and high-fiber foods that could exacerbate your symptoms. Staying hydrated is also essential, especially if you are experiencing diarrhea.

4. Monitoring Symptoms: Keep track of your symptoms, including the frequency and appearance of your stools, any changes in abdominal pain, and the presence of fever. This information will be valuable for your healthcare provider.

5. Follow-Up Care: If your symptoms worsen or if you experience additional concerning symptoms (such as increased blood in stool, severe abdominal pain, or persistent fever), seek immediate medical attention.

Conclusion
In summary, your symptoms of abdominal pain, diarrhea, fever, and blood in stool are concerning and warrant further investigation. While gastroenteritis is a possibility, the presence of blood and your family history of cancer necessitate a thorough evaluation to rule out more serious conditions. It is essential to work closely with your healthcare provider to determine the underlying cause and appropriate treatment for your symptoms.
