The glans is not firm?
Dear Dr.
Hsu,
1.
I have recently noticed that the degree of engorgement of the glans during erection is not as it used to be.
This condition has persisted for six months without improvement.
However, the hardness at the base of the penis is sufficient.
I find that the hardness of the anterior part up to the glans is inadequate.
To achieve penetration, I have to grip the base of the penis tightly, which causes the glans to enlarge and harden, allowing for successful entry.
The base of the penis feels adequately firm.
I consulted a physician who prescribed Viagra, and after taking it, the engorgement and hardness of the penis returned to previous levels.
Do I really need to rely on erectile dysfunction medication to maintain this? Are there any alternative measures for improvement?
2.
Does mitral valve prolapse affect erectile function?
Thank you.

Dr. Xu Weikai reply Urology
Glans Penis: Hello,
1.
If there is generally insufficient hardness, a penile rigidity test is required.
This test is only available at medical centers with the appropriate equipment.
As for whether long-term medication is necessary, this should be discussed with your attending physician.
2.
Mitral valve prolapse, if it does not affect cardiac output, will not impact erectile function.
However, in severe cases, it may lead to inadequate cardiac output, resulting in erectile dysfunction.
If there are any unclear points, feel free to ask again for further discussion and research.
Sincerely,
Department of Urology, Taitung Hospital, Ministry of Health and Welfare
Director Hsu Wei-Kai

More Info
Erectile dysfunction (ED) can be a complex issue, and your experience with reduced firmness in the glans (the head of the penis) while maintaining adequate hardness at the base is not uncommon. It’s important to understand that erectile function is influenced by a variety of factors, including psychological, neurological, hormonal, and vascular components.
Understanding Your Symptoms
1. Reduced Firmness in the Glans: The fact that you are experiencing a decrease in firmness specifically in the glans while the base remains firm suggests that there may be a localized issue with blood flow or nerve function. The glans is highly vascularized, and any disruption in blood flow can lead to the symptoms you are describing. This could be due to a variety of reasons, including anxiety, stress, or even physical factors such as Peyronie's disease, which involves the development of fibrous scar tissue inside the penis.
2. Use of Viagra (Sildenafil): The fact that Viagra has helped restore your erectile function indicates that the underlying issue may be related to blood flow. Viagra works by increasing blood flow to the penis, which can help achieve and maintain an erection. However, relying solely on medication may not address the root cause of your symptoms. It’s essential to consider lifestyle changes and other therapeutic options.
Potential Solutions and Lifestyle Changes
1. Lifestyle Modifications: Improving your overall health can have a significant impact on erectile function. Consider the following:
- Diet: A balanced diet rich in fruits, vegetables, whole grains, and lean proteins can improve cardiovascular health, which is crucial for erectile function.
- Exercise: Regular physical activity can enhance blood circulation and reduce stress, both of which can improve erectile function.
- Weight Management: Maintaining a healthy weight can help reduce the risk of diabetes and cardiovascular disease, both of which are linked to ED.
- Limit Alcohol and Quit Smoking: Both alcohol and tobacco can impair blood flow and contribute to erectile dysfunction.
2. Psychological Factors: If anxiety or stress is contributing to your symptoms, consider speaking with a mental health professional. Therapy can help address any underlying psychological issues that may be affecting your sexual health.
3. Medical Evaluation: Since you mentioned a history of mitral valve prolapse (MVP), it’s important to discuss this with your healthcare provider. While MVP itself is not typically associated with erectile dysfunction, any cardiovascular condition can potentially impact blood flow and overall health. A thorough evaluation by a cardiologist may be warranted to rule out any related issues.
4. Alternative Treatments: In addition to Viagra, there are other medications and treatments available for erectile dysfunction, including:
- Other PDE5 Inhibitors: Medications like Cialis (Tadalafil) and Levitra (Vardenafil) may also be effective.
- Vacuum Erection Devices: These devices can help draw blood into the penis and create an erection.
- Penile Injections or Implants: In more severe cases, these options may be considered.
Conclusion
In summary, while medications like Viagra can be effective in managing erectile dysfunction, it’s essential to explore underlying causes and consider lifestyle changes that can improve your overall health and sexual function. Consulting with a urologist or a sexual health specialist can provide you with a comprehensive evaluation and tailored treatment options. Additionally, addressing any psychological factors and maintaining open communication with your partner can also enhance your sexual experience.
