Is it safe to apply ointment while breastfeeding?
Hello Dr.
Chen, I would like to ask if it is safe to use "Ping Zhi Long" ointment long-term while breastfeeding.
The last time I visited the hospital, the doctor told me I had an anal fissure and recommended this ointment.
However, I found online that the ointment seems to contain steroids.
Will this affect my breast milk? Thank you.
CC, 30~39 year old female. Ask Date: 2011/11/04
Dr. Chen Shuzi reply Pharmacology
Hello: First, in response to questions regarding "Ping Zhi Long" ointment: 1.
Ping Zhi Long is an ointment used for the treatment of hemorrhoids, with the following ingredients: Per gram Diphenhydramine HCl 5 mg, bismuth subgallate 50 mg, Zn oxide 100 mg, benzocaine 50 mg (steroid-free).
2.
Typically, the ointment is absorbed locally through the skin and does not affect breast milk.
However, long-term use is not recommended, and it is advisable to consider discontinuing the medication once the anal fissure condition stabilizes.

When it comes to using hemorrhoid cream while breastfeeding, it is essential to consider both the ingredients of the cream and the potential effects on breast milk and the nursing infant. Hemorrhoid creams, such as "平痣隆" (which may contain ingredients like hydrocortisone or other topical steroids), are commonly prescribed for conditions like anal fissures or hemorrhoids.
Topical steroids are generally considered safe for use during breastfeeding, especially when applied to localized areas of the body, such as the anal region. The absorption of these medications through the skin is minimal, and they are unlikely to enter the bloodstream in significant amounts that would affect breast milk. However, it is always prudent to use such medications under the guidance of a healthcare professional.
Here are some key points to consider:
1. Ingredient Safety: Check the specific ingredients in the hemorrhoid cream. While many topical steroids are deemed safe during breastfeeding, some formulations may contain other active ingredients that could be of concern. If the cream contains a high potency steroid or other potentially harmful substances, it may be best to consult with your healthcare provider.
2. Application Method: When using the cream, apply it sparingly and only to the affected area. Avoid applying it to large areas of skin or using occlusive dressings, as this can increase absorption and potentially lead to systemic effects.
3. Monitoring for Side Effects: While using the cream, monitor both yourself and your baby for any unusual symptoms. If you notice any changes in your baby's behavior, feeding patterns, or skin condition, consult your pediatrician.
4. Consulting Healthcare Providers: Always discuss with your doctor or a lactation consultant before starting any new medication while breastfeeding. They can provide personalized advice based on your specific situation and the severity of your condition.
5. Alternatives: If you are concerned about using a cream with steroids, ask your healthcare provider about alternative treatments for anal fissures or hemorrhoids that may be safer during breastfeeding. Options may include dietary changes, increased fiber intake, sitz baths, or other non-steroidal topical treatments.
In summary, while using hemorrhoid cream during breastfeeding is generally considered safe, it is crucial to be aware of the specific ingredients and to use the cream as directed. Always consult with your healthcare provider for tailored advice and to ensure the safety of both you and your baby.
