Glans congestion issue?

Hello Dr.
Lin,
I would like to ask a few questions:
1.
Previously, my penis would become engorged to the point where the glans would turn purple, but I have recently noticed insufficient engorgement of the glans.
Currently, my penis can achieve an erection while sleeping, but during masturbation, the base of the penis remains somewhat firm while the glans is soft.
The same situation occurs during sexual intercourse.
Even when I do achieve an erection, there are times when it will subside despite sexual stimulation.
I do not have any poor habits or lifestyle choices, but this condition has persisted for nearly six months.
I would like to know the reason for this, and the erectile dysfunction seems to be worsening.
Are there any solutions? (I occasionally engage in hiking or swimming.)
2.
Additionally, does having sexual intercourse every day affect erectile dysfunction? I previously had more frequent sexual activity and noticed issues with hardness, but at that time, my stamina was adequate.
Later, I discovered I had sexual dysfunction and tried to reduce the frequency of sexual activity, but I found that after a prolonged period without sexual activity, my erections were even worse than when I was engaging frequently.
Is this a sign of sexual function decline?
3.
Furthermore, my semen is always a yellowish color, and even with frequent sexual activity, I cannot ejaculate white semen.
What could be the reason for this?
Thank you for taking the time to address my concerns.

Hello, sir: Thank you for your inquiry.
Here are my responses and suggestions regarding your questions:
1.
Erectile function gradually declines after puberty, depending on individual health maintenance, overall health, and physical condition to sustain optimal erectile capacity.
2.
Sexual activity should be moderate; avoid excessive frequency or prolonged periods of abstinence.
It is crucial to engage in regular exercise to maintain peak physical condition and ensure proper blood circulation throughout the body.
The penile corpora cavernosa are vascular tissues that require adequate blood flow to achieve firmness.
While erectile function naturally declines over time, it should be utilized properly rather than misused.
If you still feel that firmness is not satisfactory or duration is inadequate, it is advisable to consult a urologist for treatment.
3.
Semen is not purely white; as long as it is not blood-tinged, there should be no issue.
If there are still concerns, a semen analysis can provide clarity, so there is no need for excessive speculation.
Wishing you peace and happiness!

Understanding insufficient glans congestion and erectile issues can be quite complex, as it involves various physiological and psychological factors. Based on your description, it seems you are experiencing a combination of erectile dysfunction (ED) and issues with glans (the head of the penis) congestion. Let’s break down your concerns and provide some insights.

1. Insufficient Glans Congestion: The glans should typically become engorged with blood during arousal, leading to a firm and sensitive state. If you are noticing that the glans is not adequately filling with blood, this could be due to several reasons, including vascular issues, hormonal imbalances, or psychological factors. The fact that you can achieve an erection during sleep (nocturnal penile tumescence) suggests that the physiological mechanisms for erection are still functioning to some extent. However, the lack of firmness in the glans during sexual arousal may indicate a problem with blood flow or nerve function.

2. Erectile Dysfunction: The symptoms you describe, such as the inability to maintain an erection during sexual activity and the soft state of the glans, are indicative of erectile dysfunction. This condition can stem from various causes, including anxiety, stress, relationship issues, or underlying medical conditions such as diabetes, hypertension, or hormonal deficiencies. It is essential to consider both physical and psychological factors, as they often interplay in cases of ED.

3. Impact of Sexual Activity Frequency: Regarding your question about the frequency of sexual activity, it can indeed affect erectile function. Frequent sexual activity can lead to temporary fatigue, which might impact performance. Conversely, prolonged abstinence can also lead to decreased sexual responsiveness. The key is finding a balance that works for you. If you notice that your erectile function worsens with either frequent or infrequent sexual activity, it may be worth discussing this with a healthcare provider who specializes in sexual health.

4. Seminal Fluid Color: The color of your semen can vary based on several factors, including hydration levels, diet, and frequency of ejaculation. Yellowish semen can sometimes be a normal variation, but it can also indicate issues such as infection or inflammation. If you are consistently noticing changes in your seminal fluid, it may be worth consulting a healthcare professional to rule out any underlying conditions.

5. Recommendations: Given that you have been experiencing these issues for about six months, it would be advisable to seek a comprehensive evaluation from a urologist or a specialist in sexual medicine. They can perform necessary tests to assess blood flow, hormone levels, and any potential psychological factors contributing to your symptoms. Treatment options may include lifestyle modifications, counseling, medications, or other interventions tailored to your specific needs.

In summary, your symptoms of insufficient glans congestion and erectile difficulties could be due to a combination of physical and psychological factors. It is essential to approach this issue holistically and seek professional guidance to address both the physiological and emotional aspects of your sexual health. Remember, you are not alone in this, and many men experience similar challenges. Seeking help is a positive step toward improving your sexual health and overall well-being.
