Blood after bowel movements?
Hello, doctor.
It has been over a month since I have been experiencing bowel movements that are either hard, resembling small pellets like sheep feces, or formed into long shapes.
After passing stool, there is always bright red blood that drips down.
Sometimes, the blood comes out before the stool.
After wiping 2 to 3 times post-bowel movement, I no longer see blood.
I have researched online and believe my symptoms might indicate internal hemorrhoids, as there is no protrusion from the anus after bowel movements.
Is it possible to ignore this? My grandmother passed away from colon cancer, and in the past week, I have only gone to the bathroom twice.
Additionally, I experience a dull pain in my lower abdomen after bowel movements (I had a total hysterectomy last year).
Would increasing my fiber intake and maintaining regular bowel habits help improve this situation?

Dr. Chen Qifeng reply Gastroenterology and Hepatology
Hello: The most common cause of rectal bleeding is indeed hemorrhoids.
However, it cannot be definitively stated that it is not caused by other issues.
It is recommended to consult a colorectal surgeon.
Wishing you good health, Dr.

More Info
Experiencing blood after bowel movements can be alarming and is often indicative of an underlying issue that requires attention. Based on your description, it seems that you have been dealing with this symptom for over a month, with fresh red blood appearing after bowel movements, sometimes even before the stool is expelled. You also mentioned a family history of colorectal cancer, which adds an important layer to your situation.
The presence of bright red blood in your stool or on toilet paper can be attributed to several potential causes, the most common of which include hemorrhoids (particularly internal hemorrhoids), anal fissures, or more serious conditions such as diverticular disease or colorectal polyps. Given your symptoms and the fact that you have experienced changes in your bowel habits, it is crucial to approach this situation with caution.
1. Hemorrhoids: These are swollen veins in the lower rectum and anus that can cause bleeding, especially during bowel movements. They can be internal (inside the rectum) or external (under the skin around the anus). If you are experiencing hard stools, this can lead to straining during bowel movements, which can exacerbate hemorrhoids and lead to bleeding.
2. Anal Fissures: These are small tears in the lining of the anus, often caused by passing hard or large stools. They can be very painful and may cause bleeding during or after a bowel movement.
3. Colorectal Conditions: Given your family history of colorectal cancer, it is essential to consider other potential causes such as polyps or inflammatory bowel disease. Although these conditions are less common, they can present with similar symptoms and should not be overlooked.
4. Dietary Factors: You mentioned the possibility of improving your diet with more fiber. Increasing fiber intake can help soften stools and promote regular bowel movements, which may alleviate some of the straining and discomfort associated with hard stools. Foods high in fiber include fruits, vegetables, whole grains, and legumes. However, it is important to increase fiber gradually and ensure adequate hydration to prevent further gastrointestinal discomfort.
5. Medical Evaluation: Given the duration of your symptoms, the presence of blood, and your family history, it is highly advisable to seek a thorough evaluation from a healthcare professional. A gastroenterologist can perform necessary diagnostic tests, such as a colonoscopy, to rule out any serious conditions and provide appropriate treatment options.
In summary, while dietary changes can certainly help improve bowel regularity and potentially reduce symptoms, it is crucial to address the bleeding and any associated discomfort with a healthcare provider. Ignoring these symptoms could lead to complications or delay the diagnosis of a more serious condition. Early intervention is key, especially considering your family history of colorectal cancer. Please prioritize scheduling an appointment with a medical professional to discuss your symptoms in detail and receive the appropriate care.
