Is Long-Term Use of Jianwei Xian Safe? Side Effects to Consider - Gastroenterology and Hepatology

Is it safe to take Jianwei Xian for a long time? Are there any side effects?

Hello, Doctor.
I often experience stomach pain after lunch and dinner.
Taking 1 to 2 tablets of a digestive aid (ingredients: Aluminum Hydroxide Dried Gel 200mg, Magnesium Hydroxide 200mg, Simethicone 25mg) provides relief.
However, a pharmacist recently informed me that this digestive aid should not be taken regularly.
I would like to ask: 1.
Is it safe to use this digestive aid long-term? 2.
Besides the possibility of constipation, are there any other side effects?

Hello: There are several issues to discuss regarding the long-term use of antacids.
First, a correct diagnosis should be established to avoid masking the underlying cause of the condition.
In some cases of gastric cancer, the use of acid suppressants may lead to partial symptom relief.
The medication itself may cause side effects such as constipation, nausea, vomiting, poor appetite, weakness, and allergic symptoms like skin itching.
Additionally, there are reports indicating that long-term use of aluminum-containing antacids may be associated with an increased risk of dementia.
Wishing you good health, Dr.

When considering the long-term use of Jianwei Xian (健胃仙), which contains Aluminum Hydroxide, Magnesium Hydroxide, and Simethicone, it is essential to understand both its therapeutic benefits and potential side effects.
1. Long-Term Use of Jianwei Xian: While Jianwei Xian can be effective for alleviating symptoms of gastric discomfort, such as heartburn or indigestion, it is generally not recommended for long-term use without medical supervision. The primary reason for this caution is due to the active ingredients. Aluminum Hydroxide and Magnesium Hydroxide are antacids that neutralize stomach acid, providing quick relief. However, prolonged use can lead to an imbalance in electrolytes and other complications.

2. Potential Side Effects:
- Constipation: As you mentioned, one of the common side effects of Aluminum Hydroxide is constipation. This is particularly relevant for individuals who may already have a predisposition to gastrointestinal issues.

- Diarrhea: Conversely, Magnesium Hydroxide can cause diarrhea, especially if taken in excess. The balance between these two ingredients can affect bowel movements, leading to alternating episodes of constipation and diarrhea.

- Electrolyte Imbalance: Long-term use of antacids can lead to an accumulation of aluminum in the body, particularly in individuals with compromised kidney function. This can result in conditions such as osteomalacia (softening of the bones) or other metabolic bone diseases.

- Kidney Issues: For individuals with pre-existing kidney conditions, the use of antacids containing aluminum can exacerbate kidney problems and lead to further complications.

- Masking Underlying Conditions: Regular use of antacids may mask symptoms of more serious gastrointestinal conditions, such as peptic ulcers or gastroesophageal reflux disease (GERD). This could delay diagnosis and treatment of these conditions.

Given these considerations, it is advisable to consult with a healthcare professional regarding the long-term use of Jianwei Xian. If you find yourself needing to take it frequently, it may be worth exploring the underlying causes of your gastric discomfort.
In addition to medication, lifestyle modifications can also play a significant role in managing gastric issues. These may include dietary changes, such as avoiding spicy or acidic foods, eating smaller meals, and not lying down immediately after eating. Stress management techniques, such as mindfulness or yoga, can also help alleviate symptoms.

In summary, while Jianwei Xian can provide temporary relief from gastric discomfort, it is not advisable to use it long-term without medical guidance due to potential side effects and the risk of masking more serious conditions. If you experience persistent gastric pain, it is crucial to seek a thorough evaluation from a healthcare provider to determine the underlying cause and appropriate treatment options.
