Nasal congestion in the morning, with the possibility of nosebleeds?

Hello, doctor.
The weather has been dry and snowy lately.
A couple of days ago, while I was walking, I had a bit of a nosebleed (on the left side), but it stopped afterward.
However, the next morning, I noticed some blood when I blew my nose due to nasal congestion.
The following day, the issue improved, but I still experienced nasal congestion in the morning.
About two days later (which is today), I had another nosebleed, and this time, there was a significant amount of blood mixed with mucus.
When I used a tissue to clean my nose, I also noticed blood.
Am I experiencing some kind of problem? I'm really anxious because I've never had this happen before.

Dear Kevin:
Hello, the causes of nosebleeds are mostly due to bleeding from the anterior nasal septum or the inferior turbinate.
After a wound forms, it usually takes a few days to heal, during which scabbing may occur, leading to nasal congestion.
Forcefully blowing the nose can also cause re-bleeding, and some of the blood may flow back and form blood-tinged sputum.
Most nosebleeds occur due to the rupture of the mucosa and typically resolve on their own.
However, if bleeding persists, it is advisable to seek medical attention for a thorough examination.

Wishing you safety and health,

More Info

Morning nasal congestion accompanied by bleeding can be concerning, especially if it is a new symptom for you. Based on the information provided, there are several potential causes for your symptoms, and it is essential to consider both environmental factors and underlying medical conditions.

Firstly, the dry weather and snow you mentioned can significantly contribute to nasal issues. Dry air can lead to the drying out of the nasal mucosa, making it more susceptible to irritation and bleeding. When you blow your nose, especially if done forcefully, it can cause small blood vessels in the nasal passages to rupture, leading to the presence of blood in your mucus. This is particularly common in winter months when indoor heating can further dry out the air.

The fact that you are experiencing nasal congestion primarily in the morning could also indicate that you are dealing with allergic rhinitis or sinusitis. Allergens or irritants in your environment, such as dust, pet dander, or even mold, can trigger inflammation in the nasal passages, leading to congestion. Additionally, sinusitis, which is an inflammation of the sinuses, can cause similar symptoms, including nasal congestion and bleeding.

Another consideration is the possibility of structural issues within your nasal passages, such as a deviated septum or nasal polyps. These conditions can obstruct airflow and lead to chronic congestion, which may exacerbate the likelihood of bleeding when you blow your nose. If you have a history of frequent nasal congestion or sinus infections, these structural issues might be worth investigating.

Moreover, the presence of blood in your phlegm (sputum) raises additional concerns. While it may be related to nasal bleeding, it could also indicate a more serious issue involving the lungs or respiratory system. Conditions such as bronchitis, pneumonia, or even more severe issues like pulmonary embolism or lung cancer can present with blood in the sputum. Given that you mentioned this is a new symptom, it is crucial to seek medical attention to rule out any serious underlying conditions.

In summary, the combination of dry air, potential allergies, structural issues, and the presence of blood in your sputum suggests that a thorough evaluation by a healthcare professional is warranted. An ear, nose, and throat (ENT) specialist can perform a detailed examination of your nasal passages, while a pulmonologist may be needed to assess your respiratory health. Diagnostic tests such as nasal endoscopy, imaging studies, or allergy testing may be recommended based on your symptoms.

It is understandable to feel anxious about these symptoms, especially since they are new to you. However, seeking medical advice will help clarify the cause of your symptoms and guide you toward appropriate treatment options. In the meantime, consider using a humidifier in your home to add moisture to the air, which may help alleviate some of the dryness in your nasal passages. Additionally, staying hydrated and using saline nasal sprays can help keep your nasal mucosa moist and reduce irritation.
