Hello, I would like to ask about issues related to frequent urination and residual urine?
Hello, Doctor.
I have been troubled by frequent urination since middle school, and now as a junior in college, I am really worried about how this will affect my military service.
Sometimes I feel the urge to urinate every 5 to 10 minutes, while other times I can go for 2 to 3 hours without needing to.
Additionally, after urinating, I sometimes experience a feeling of incomplete emptying and have to go 2 to 3 more times to relieve the urgency (the volume of urine is normal; it’s not just a little bit.
I also don’t drink much water, usually not exceeding 2000 cc per day, yet I produce a large volume each time.
I am truly perplexed by this).
I have undergone examinations, and there is no inflammation in the urethra, and the uroflowmetry test results are normal.
Could this be related to my constitution, or am I possibly a patient with interstitial cystitis, or could it be something like sympathetic nervous system dysregulation? I do tend to get anxious easily, so could it be an issue with nerve control? Please help me, Doctor.
Are there any other symptoms I should be aware of, and what kind of tests would you recommend? I live in Taichung, so if you have any recommendations for clinics or doctors, I would greatly appreciate it.
Thank you in advance for your response.

More Info
Frequent urination and the sensation of residual urine can be distressing symptoms that significantly impact one's quality of life. Based on your description, it seems you have been experiencing these symptoms for a considerable time, and despite undergoing some tests, the underlying cause remains unclear. Let's explore the potential causes and solutions for your situation.
Understanding Frequent Urination and Residual Urine
1. Physiological Factors:
- Bladder Sensitivity: Some individuals have a more sensitive bladder, which can lead to frequent urges to urinate even when the bladder is not full. This can be exacerbated by anxiety or stress, which you mentioned is a concern for you.
- Overactive Bladder (OAB): This condition is characterized by a sudden urge to urinate that may be difficult to control. It can occur with or without incontinence and is often associated with increased urinary frequency and urgency.
- Interstitial Cystitis (IC): This is a chronic condition that causes bladder pressure, bladder pain, and sometimes pelvic pain. It can lead to frequent urination and a feeling of urgency, often accompanied by discomfort.
2. Neurological Factors:
- Nerve Dysfunction: The bladder is controlled by a complex network of nerves. If there is any dysfunction in these nerves, it could lead to symptoms like frequent urination and a feeling of incomplete emptying.
- Sympathetic Nervous System: Anxiety and stress can activate the sympathetic nervous system, which may affect bladder function and lead to increased urinary frequency.
3. Psychological Factors:
- Anxiety and Stress: As you noted, you are prone to anxiety. Psychological factors can significantly influence urinary habits. Anxiety can lead to hypervigilance regarding bodily sensations, making you more aware of the need to urinate.
4. Other Medical Conditions:
- Diabetes: Uncontrolled diabetes can lead to increased urination due to high blood sugar levels. It’s essential to rule this out, especially if you have other symptoms like increased thirst.
- Urinary Tract Infections (UTIs): Although you mentioned that your urine tests showed no inflammation, UTIs can sometimes present with atypical symptoms.
Suggested Investigations
Given your symptoms and the fact that initial tests have not provided clear answers, further investigations may be warranted:
- Urodynamic Studies: These tests assess how well the bladder and urethra are functioning. They can help determine if there is an overactive bladder or other functional issues.
- Cystoscopy: This procedure allows direct visualization of the bladder and urethra, which can help identify any abnormalities.
- Pelvic Floor Assessment: A physical therapy evaluation may help determine if pelvic floor dysfunction is contributing to your symptoms.
Management Strategies
1. Behavioral Techniques:
- Bladder Training: This involves gradually increasing the time between urination to help train your bladder to hold more urine.
- Pelvic Floor Exercises: Strengthening the pelvic floor muscles can help improve bladder control.
2. Medications:
- If diagnosed with OAB or IC, medications such as anticholinergics or beta-3 adrenergic agonists may be prescribed to help manage symptoms.
3. Lifestyle Modifications:
- Reducing caffeine and alcohol intake, as these can irritate the bladder.
- Keeping a bladder diary to track fluid intake and urination patterns can provide valuable information for your healthcare provider.
Seeking Medical Help
Since you are located in Taichung, I recommend seeking a urologist who specializes in bladder disorders. Look for clinics or hospitals with a good reputation for treating urological conditions. It may also be beneficial to consult with a psychologist or counselor if anxiety is a significant factor in your symptoms.
In conclusion, frequent urination and the sensation of residual urine can stem from various causes, including physiological, neurological, and psychological factors. A comprehensive approach involving further testing, behavioral strategies, and possibly medication can help manage your symptoms effectively. Don't hesitate to reach out to a healthcare professional for personalized advice and treatment options.
