Mild Fever After Surgery: When to Worry? - Family Medicine

Slight fever

Hello, Dr.
Liao.
I am asking a question on behalf of my mother.
She was hospitalized for an open fracture and underwent surgery before the Lunar New Year, and she was discharged to rest at home before the holiday.
Recently, over the past two days, we have measured her temperature (axillary) and found it to be slightly elevated, around 36.5 to 37.4 degrees Celsius.
My mother does not have any discomfort.
Is this what is referred to as a low-grade fever? Is there a possibility that this could be a fever due to wound infection? Thank you!

Kenny, 70~79 year old female. Ask Date: 2012/02/19

Dr. Liao Yaopan reply Family Medicine

Hello: Regarding the changes in body temperature, it is indeed possible to experience a mild fever at times.
The potential causes need to be clarified, so it is advisable to visit a hospital for a personal examination by a physician.

Hello! Thank you for your question regarding your mother's mild fever after surgery. It's understandable to be concerned about any changes in temperature following a surgical procedure, especially after an open fracture repair.

First, let's clarify what constitutes a mild fever. Typically, a normal body temperature ranges from about 36.1°C to 37.2°C (97°F to 99°F). A temperature above this range, particularly between 37.3°C and 38.3°C (99°F to 100.9°F), is often considered a mild fever. Your mother's recorded temperatures of 36.5°C to 37.4°C fall within this range, indicating that she may indeed be experiencing a mild elevation in temperature.

Post-operative fever can occur for various reasons, and it is essential to consider the context of her surgery and overall health. In the immediate post-operative period, fever can be a common response to the surgical stress and inflammation. This is often referred to as a "physiological response" and may not necessarily indicate an infection.
However, it is crucial to monitor the situation closely. While a mild fever can be benign, there are specific signs that may suggest a more serious issue, such as an infection. These include:
1. Increased Temperature: If her temperature rises above 38.3°C (100.9°F) or persists over several days, it may warrant further investigation.

2. Signs of Infection: Look for redness, swelling, warmth, or discharge at the surgical site. Additionally, systemic signs such as chills, increased heart rate, or changes in mental status should be noted.

3. Pain or Discomfort: If she begins to experience increased pain at the surgical site or any new symptoms, it could indicate a complication.

In the case of an open fracture, there is a risk of infection due to the exposure of the bone and surrounding tissues. Surgical site infections can develop days to weeks after surgery, so vigilance is essential. If you notice any concerning symptoms, such as worsening fever, increased pain, or changes in her overall condition, it is advisable to contact her healthcare provider promptly.

In summary, while your mother's current temperature readings suggest a mild fever, it is essential to monitor her closely for any changes. If her fever persists or if any signs of infection develop, seeking medical advice is crucial. It’s always better to err on the side of caution when it comes to post-operative care.
Encourage her to stay hydrated, rest, and maintain follow-up appointments with her healthcare provider to ensure a smooth recovery. If you have any further questions or concerns, do not hesitate to reach out to her medical team. Wishing your mother a speedy recovery!
