Recently, there have been elevated levels of triglycerides and fatty liver?
Hello Doctor,
Recently, I'm not sure if it's due to my own excessive stress or if there's a real issue with my body, but I've been experiencing fatty diarrhea for four consecutive days.
I only found out today that it was fatty diarrhea because for the previous four days, my stools were watery and lighter in color than usual, appearing a pale yellow-brown.
I have already seen a doctor who prescribed me a bowel regulator, and today my stools have returned to a more formed shape, although they are still somewhat circular.
Additionally, I sometimes feel a dull discomfort in my upper right abdomen, but it's not painful.
A few days ago, I underwent a health check-up, including a urinalysis and blood tests, which showed elevated triglyceride levels.
Could this be related to the fatty diarrhea I've been experiencing? I also had an abdominal ultrasound, and both my liver and kidneys appeared normal.
I've been exercising more recently; will this situation improve?

Dr. Yang Zhichao reply Gastroenterology and Hepatology
Hello Mr./Ms.
L, steatorrhea refers to watery stools that have a layer of floating oil on top, commonly seen in patients with chronic pancreatitis.
A stool test would show a positive fat reaction.
If the above conditions are met, it should be classified as steatorrhea.
At 21 years old, it is more likely that you are experiencing intestinal dysbiosis or gastrointestinal dysfunction, which are related to lifestyle habits, emotions, and stress.
If there is no family history of cancer, I suspect you fall into this category.
Additionally, elevated triglycerides are generally not related to gastrointestinal symptoms.
Engaging in more physical activity and consuming fruits, vegetables, and adequate hydration would be beneficial.
If your symptoms persist without improvement, I recommend visiting a nearby medical facility for further examination.
Thank you for your inquiry.
Sincerely, Dr.
Yang Chih-Chao, Changhua Hospital.

More Info
High triglycerides and fatty diarrhea can be concerning symptoms that may indicate underlying health issues. Triglycerides are a type of fat found in your blood, and elevated levels can be linked to various health problems, including cardiovascular disease and metabolic syndrome. In your case, the high triglyceride levels, coupled with the recent episodes of fatty diarrhea, warrant a closer look at your overall health and lifestyle.
Understanding High Triglycerides
Triglycerides are primarily derived from the food we eat, particularly carbohydrates and fats. When you consume more calories than your body needs, especially from sugary or fatty foods, the excess calories are converted into triglycerides and stored in fat cells. High levels of triglycerides can be caused by several factors, including obesity, physical inactivity, excessive alcohol consumption, and a diet high in carbohydrates and sugars.
A triglyceride level above 150 mg/dL is considered borderline high, while levels above 200 mg/dL are high, and levels above 500 mg/dL are very high. Your reported level of 797 mg/dL is significantly elevated and could increase your risk of pancreatitis, a serious condition that requires immediate medical attention.
Fatty Diarrhea
Fatty diarrhea, or steatorrhea, occurs when your body is unable to properly digest and absorb fats from the food you eat. This can lead to oily, foul-smelling stools that may float. The condition can be caused by several factors, including malabsorption syndromes, pancreatic insufficiency, or liver disease. Given that you have experienced this symptom alongside high triglycerides, it may indicate that your body is struggling to process fats effectively.
Potential Connections
The connection between high triglycerides and fatty diarrhea could be related to several factors:
1. Pancreatic Function: If your pancreas is not producing enough digestive enzymes, it can lead to malabsorption of fats, resulting in fatty diarrhea. High triglyceride levels can also indicate that the pancreas is under stress.
2. Liver Health: The liver plays a crucial role in metabolizing fats. If there are issues with liver function, it can affect how triglycerides are processed and lead to digestive problems.
3. Dietary Factors: A diet high in fats and sugars can contribute to both high triglycerides and digestive issues. If your recent eating habits have included high-fat foods, this could explain both your triglyceride levels and your gastrointestinal symptoms.
Recommendations for Improvement
1. Dietary Changes: Adopt a heart-healthy diet that is low in saturated fats, trans fats, and sugars. Focus on whole grains, fruits, vegetables, lean proteins, and healthy fats (like those from fish, nuts, and olive oil). Reducing carbohydrate intake, particularly refined carbs and sugars, can also help lower triglyceride levels.
2. Regular Exercise: Incorporating regular physical activity into your routine can significantly improve your triglyceride levels and overall health. Aim for at least 150 minutes of moderate-intensity aerobic exercise each week.
3. Weight Management: If you are overweight, losing even a small percentage of your body weight can help lower triglyceride levels and improve your overall health.
4. Limit Alcohol: If you consume alcohol, consider reducing your intake, as it can contribute to high triglyceride levels.
5. Medical Follow-Up: Since you have already consulted a physician and received treatment, it is essential to continue monitoring your symptoms and triglyceride levels. If fatty diarrhea persists or worsens, further evaluation may be necessary to rule out any underlying conditions.
6. Hydration and Fiber: Ensure you are drinking plenty of water and consuming enough dietary fiber to support digestive health.
In conclusion, while high triglycerides and fatty diarrhea can be concerning, making lifestyle changes and following your doctor's recommendations can significantly improve your health. Regular follow-ups with your healthcare provider will also help monitor your progress and address any ongoing issues.
