Post-Surgery Discomfort After Varicocele Repair: Causes and Solutions - Urology

Varicocele surgery

I have no symptoms of varicocele, and I only discovered it during infertility checks after marriage.
I underwent surgery for varicocele treatment in early 2010, but recently I have been experiencing a dull pain on both sides of my lower abdomen during sexual intercourse, which continues for several hours after the activity.
(Masturbation does not cause pain.) In fact, I noticed a slight discomfort in that area after starting sexual activity two years post-surgery, but it was mild and I didn't pay much attention to it.
However, the condition seems to have worsened recently.
What could be the cause? Thank you.

A scrotal ultrasound is required for diagnosis.

Post-surgery discomfort following varicocele repair can be a complex issue, particularly when the symptoms manifest during sexual activity. Varicocele repair is generally performed to alleviate issues related to enlarged veins in the scrotum, which can affect fertility and cause discomfort. However, it is not uncommon for patients to experience discomfort or pain even after the surgery has been deemed successful.

There are several potential reasons for the discomfort you are experiencing:
1. Nerve Sensitivity: After any surgical procedure, the nerves in the area may become hypersensitive. This can lead to discomfort during activities that involve the lower abdomen, including sexual intercourse. The nerves may take time to heal, and this sensitivity can sometimes persist for months or even longer.

2. Scar Tissue Formation: Surgery can lead to the formation of scar tissue, which may cause tightness or discomfort in the surrounding tissues. This scar tissue can sometimes adhere to nerves or other structures, leading to pain during certain movements or activities.

3. Muscle Tension: The pelvic floor muscles can become tense or strained, particularly if there is anxiety or apprehension about pain during sexual activity. This tension can contribute to discomfort during intercourse.

4. Infection or Inflammation: Although less common, it is possible that there could be an underlying infection or inflammation in the surgical area that could be causing pain. If the pain is accompanied by other symptoms such as swelling, redness, or fever, it is essential to consult your physician.

5. Psychological Factors: Sometimes, the anticipation of pain can lead to increased tension and discomfort during sexual activity. Psychological factors can play a significant role in how pain is perceived and can exacerbate physical symptoms.

6. Underlying Conditions: It is also important to consider whether there are any other underlying medical conditions that could be contributing to your discomfort. Conditions such as pelvic floor dysfunction or other urological issues may need to be evaluated.

Given that your discomfort has increased over time, it is advisable to consult with your urologist or a specialist in sexual medicine. They can perform a thorough evaluation, which may include a physical examination, imaging studies, or nerve conduction studies to determine the exact cause of your pain.
In terms of management, several approaches may be beneficial:
- Physical Therapy: A pelvic floor physical therapist can help address muscle tension and improve flexibility, which may alleviate discomfort during sexual activity.

- Pain Management: Your physician may recommend medications to help manage pain, including non-steroidal anti-inflammatory drugs (NSAIDs) or other analgesics.

- Counseling: If psychological factors are contributing to your discomfort, speaking with a therapist who specializes in sexual health may be beneficial.

- Follow-Up: Regular follow-up with your healthcare provider is essential to monitor your symptoms and adjust treatment as necessary.

In conclusion, while post-surgery discomfort after varicocele repair can be concerning, there are various potential causes and solutions. It is crucial to communicate openly with your healthcare provider about your symptoms to ensure appropriate management and support.
