Post-ejaculation issues?
Hello, Dr.
Hsu.
I have a strong sexual drive, so I experience ejaculation once a day or every two days.
Immediately after ejaculation, I do not feel fatigued, but the next day I experience lower back pain, specifically on the right side.
This lower back pain occurs whenever I ejaculate, and the volume of ejaculation is normal.
Is there a correlation between ejaculation and lower back pain? Thank you.

A-Chung: Hello, it should be fine.
It might be due to a longer duration of erection, which can lead to prolonged nerve stimulation, consequently causing the associated muscles to become more fatigued and resulting in soreness.
As for why it's occurring on the right side, it could be related to the position during intercourse.

Ejaculation and lower back pain can indeed be linked, although the relationship is not straightforward and can vary from person to person. The experience of lower back pain following ejaculation, particularly if it is consistently located on one side, may be influenced by several factors, including muscular tension, nerve irritation, and even psychological aspects.
Firstly, during sexual activity and ejaculation, various muscle groups are engaged, including those in the pelvic region, lower back, and abdomen. If there is pre-existing tension or weakness in these muscles, the physical exertion associated with sexual activity could lead to muscle strain or discomfort afterward. This is particularly relevant if the individual has a sedentary lifestyle or poor posture, which can contribute to muscle imbalances and tension in the lower back.
Additionally, the act of ejaculation involves a series of contractions in the pelvic floor muscles. If these muscles are overly tight or if there is an underlying condition such as pelvic floor dysfunction, it could lead to referred pain in the lower back. This is because the nerves that innervate the pelvic region also supply the lower back, and irritation or tension in one area can manifest as pain in another.
Another consideration is the role of the nervous system. The sympathetic nervous system, which is activated during sexual arousal and ejaculation, can lead to increased muscle tension. If an individual is experiencing heightened anxiety or stress, this can exacerbate muscle tension and contribute to pain. In some cases, psychological factors such as anxiety about sexual performance or other stressors can lead to physical symptoms, including lower back pain.
It's also worth noting that certain medical conditions can contribute to both ejaculation and lower back pain. For example, conditions such as prostatitis or other pelvic inflammatory conditions can lead to discomfort during or after ejaculation, which may be perceived as lower back pain. If there are any underlying health issues, it is essential to address those with a healthcare provider.
In summary, while there can be a connection between ejaculation and lower back pain, it is essential to consider the broader context, including muscular, neurological, and psychological factors. If this pain persists or worsens, it would be advisable to consult a healthcare professional for a thorough evaluation. They may recommend physical therapy to address any muscular imbalances, stress management techniques, or further investigation into any underlying medical conditions. Keeping a symptom diary, noting the frequency and intensity of pain in relation to sexual activity, can also provide valuable information for your healthcare provider.
