Semen cyst
Hello doctor, I have noticeable painless cystic masses above both of my testicles.
Sometimes, when I watch adult films, they seem to swell even larger and there is some dull pain.
Could this be a spermatocele? If I have spermatocele on both sides, will it affect my future fertility? Thank you.
Daxuesheng, 20~29 year old female. Ask Date: 2012/03/22
Dr. Xu Weikai reply Urology
University Student: There is a possibility of that, but a scrotal ultrasound is still needed to confirm the diagnosis.
However, it is rare for a spermatocele to change in size due to erection or arousal.
Generally, a spermatocele, if there is no inflammation or other complications, rarely affects fertility.
If you are concerned, you can start with a semen analysis for preliminary evaluation.
If there are any unclear points, feel free to ask again for further discussion and research.

More Info
Seminal cysts, also known as spermatic cysts or epididymal cysts, are fluid-filled sacs that can develop in the epididymis, which is the coiled tube located at the back of each testicle. These cysts are generally benign and often asymptomatic, meaning they do not usually cause pain or discomfort. However, they can sometimes lead to discomfort or a dull ache, particularly if they become larger or if there is increased pressure in the area, such as during sexual arousal.
Causes of Seminal Cysts
The exact cause of seminal cysts is not fully understood, but they are thought to arise from blockages in the ducts that transport sperm. Factors that may contribute to the formation of these cysts include:
1. Injury or Trauma: Previous injuries to the testicular area can lead to the development of cysts.
2. Infection: Infections in the reproductive system may also contribute to the formation of cysts.
3. Congenital Factors: Some individuals may be born with a predisposition to develop these cysts.
Symptoms
As you mentioned, seminal cysts are typically painless and may not present any symptoms. However, in some cases, individuals may experience:
- A noticeable lump or swelling above the testicle.
- A feeling of heaviness or dull pain in the scrotum, especially during sexual arousal or physical activity.
- Discomfort when pressure is applied to the area.
Impact on Fertility
In most cases, seminal cysts do not significantly impact fertility. They are usually benign and do not interfere with sperm production or transport. However, if the cysts are large or if they cause significant discomfort, they may require medical evaluation and possible treatment.
If you are experiencing discomfort or if the cysts are causing anxiety regarding your fertility, it is advisable to consult a healthcare professional. They may perform a physical examination and possibly an ultrasound to confirm the diagnosis and rule out other conditions.
When to Seek Medical Attention
You should seek medical attention if you experience any of the following:
- Sudden increase in size of the cysts.
- Severe pain or discomfort in the testicular area.
- Changes in urination or sexual function.
- Any other unusual symptoms that concern you.
Conclusion
In summary, while seminal cysts can be concerning, they are often benign and do not typically affect fertility. However, if you have any doubts or if your symptoms change, it is essential to consult a healthcare provider for a thorough evaluation. They can provide you with the necessary information and reassurance regarding your condition and its potential impact on your reproductive health.
