Palpitations, chest tightness, and nausea at midnight?
Hello, doctor! For the past two weeks, I have been experiencing intense heart palpitations almost every night after midnight.
It feels like something is trying to jump out from my chest and mouth, and my heart rate increases significantly.
Additionally, I have difficulty breathing, and taking deep breaths makes me feel nauseous.
This occurs more than five times a night (I don't work and go to bed late).
I have actually experienced these symptoms since before I turned 30, but not frequently.
I initially thought it was a stomach issue (I have a history of chronic gastritis) because I often feel quite hungry when these symptoms occur, but even after eating, I still experience palpitations.
Thank you for your help! Wishing you and your family peace and happiness!

Hello, it sounds like external stimuli such as hunger and gastroesophageal reflux may be causing autonomic nervous system dysregulation, leading to issues with heart rhythm and symptoms like palpitations.
The combined use of antiarrhythmic medications and some gastrointestinal medications may help improve your condition.

Nighttime palpitations can be a concerning symptom that many individuals experience, and understanding the underlying causes is essential for finding effective solutions. Based on your description, it seems that you are experiencing episodes of rapid heartbeats and shortness of breath, which can be distressing, especially when they occur frequently during the night.
There are several potential causes for nighttime palpitations:
1. Anxiety and Stress: Anxiety is a common trigger for palpitations. If you are feeling anxious or stressed, especially at night, it can lead to an increased heart rate and feelings of breathlessness. The body's fight-or-flight response can be activated, causing these symptoms.
2. Gastroesophageal Reflux Disease (GERD): Given your history of chronic gastritis, it is possible that acid reflux could be contributing to your symptoms. GERD can cause irritation of the esophagus, leading to sensations that may be interpreted as palpitations or heart discomfort.
3. Caffeine and Stimulants: Consumption of caffeine or other stimulants, even in the hours leading up to bedtime, can lead to increased heart rate and palpitations. If you consume coffee, tea, energy drinks, or certain medications that contain stimulants, consider reducing or eliminating them from your evening routine.
4. Hormonal Changes: Hormonal fluctuations, particularly in women, can lead to palpitations. Conditions such as premenstrual syndrome (PMS) or menopause can cause changes in heart rhythm.
5. Sleep Disorders: Conditions like sleep apnea can lead to nighttime palpitations. Sleep apnea causes interruptions in breathing during sleep, which can result in increased heart rate and feelings of panic or breathlessness.
6. Electrolyte Imbalances: An imbalance in electrolytes, such as potassium or magnesium, can affect heart rhythm. If you have dietary restrictions or are not consuming a balanced diet, this could be a contributing factor.
7. Underlying Heart Conditions: While less common, it is important to rule out any underlying heart conditions, especially if you have a history of heart issues or if the palpitations are accompanied by other concerning symptoms such as chest pain or fainting.
Given that you have experienced these symptoms for several years, it is crucial to consult with a healthcare professional for a thorough evaluation. They may recommend tests such as an electrocardiogram (ECG), echocardiogram, or Holter monitor to assess your heart's rhythm and function. Additionally, discussing your symptoms with a doctor can help determine if further investigation into anxiety, gastrointestinal issues, or other potential causes is warranted.
In the meantime, here are some strategies you can implement to help manage your symptoms:
- Practice Relaxation Techniques: Engaging in relaxation exercises such as deep breathing, meditation, or yoga can help reduce anxiety and stress, potentially alleviating palpitations.
- Maintain a Regular Sleep Schedule: Try to establish a consistent sleep routine, going to bed and waking up at the same time each day. This can help regulate your body's internal clock.
- Limit Stimulants: Reduce or eliminate caffeine and other stimulants, especially in the hours leading up to bedtime.
- Monitor Your Diet: Pay attention to your eating habits, particularly in the evening. Avoid heavy meals close to bedtime, and consider keeping a food diary to identify any potential triggers.
- Stay Hydrated: Ensure you are drinking enough water throughout the day, as dehydration can contribute to palpitations.
- Seek Professional Help: If your symptoms persist or worsen, it is essential to seek medical advice. A healthcare professional can provide personalized recommendations and treatment options based on your specific situation.
In conclusion, while nighttime palpitations can be alarming, understanding their potential causes and implementing lifestyle changes can help manage the symptoms. Consulting with a healthcare provider is crucial for a comprehensive evaluation and tailored treatment plan.
